Chartwell is currently hiring a Home Infusion Nurse to coordinate and perform skilled nursing care for patients in the Williamsport, PA region. Chartwell's Home Infusion Services provides patients with individualized infusion therapy assistance and instruction, primarily in the comfort and convenience of their own homes. Our program—for people of all ages who need to receive medications or nutrients through a vein, under the skin, in the spine or through a feeding tube—is designed to help patients become as independent as possible after they are discharged from a hospital or other medical facility.

As a Home Infusion Nurse, you will also teach patients and significant others home medical management; and provide leadership by working cooperatively with staff to establish priorities and goals of patient care.

This casual position will work on an on-call basis from 4:30 PM - 10:00 PM. Home visits are conducted within the Williamsport, PA region. 

  • Attend scheduled patient care conferences, in-services, departmental meetings, etc.
  • Communicate to supervisor, physicians and other health care providers, as appropriate, any changes in the patient's condition.
  • Communicate with others in a warm and helpful manner while simultaneously building credibility and rapport.
  • Complete all required patient care documentation and reports in a timely manner.
  • Complete work on schedule.
  • Comply with all applicable company policies and procedures.
  • Demonstrate ability to work effectively and maintain expected productivity.
  • Demonstrate reliability and follow-through on all assigned tasks.
  • Display a neat, clean, professional appearance at all times.
  • Maintain confidentiality of patient information.
  • Maintain courteous demeanor at all times.
  • Maintain professional licensure, certification and health requirements as well as demonstrating clinical competence of orientation programs, etc.
  • Make sure that work meets quality standards.
  • Participate in Chartwell Pennsylvania's Performance Improvement Program.
  • Participate in the activities of the Performance Improvement Program.
  • Participate in the orientation of other nurses or center personnel, as requested.
  • Promote Chartwell culture by providing quality, comprehensive services to all customers through a team approach.Demonstrates ability to communicate effectively and express ideas clearly.
  • Provide infusion and other skilled nursing care to patients.
  • Provide pre-discharge patient assessments when appropriate.
  • Represent self in a professional and ethical; manner at all times.
  • Set high goals or standards of performance for self.
  • Show pride in work.
  • Submit all required patient care documentation and reports in a timely manner.
  • Support the marketing efforts of the center by assisting the sales team and providing educational programs to referral sources, as requested.
  • Take initiative to present ideas/ suggestion to management.
  • Take on call responsibility as assigned to provide 24 hour availability, and provide these services in accordance with established protocols.
  • Take professional responsibility for self-protection during required work in potentially hazardous situations (i.e., handling of cytoxic agents, handling of blood and body fluids).
  • Utilize appropriate communication lines in relaying problems, concerns, questions, and ideas.


  • Diploma from an accredited school of nursing required. B.S.N. preferred. 
  • 1 year clinical experience involving infusion therapy, including patient assessment skills, and venous access skills
  • 1 year previous specialized home health experience preferred. 
  • CPR certification required. PICC training preferred. 
  • Excellent interpersonal, communication and organizational skills required. 
  • Ability to assume a flexible work schedule.

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 34 Criminal Clearance with Renewal
