Description

Purpose:
Provides Remote Monitoring Assessment to patients within their home setting under the direction of the Disease State Management Coordinator.

Responsibilities:
  • Accepts and immediately documents verbal orders per policy
  • Accurately and effectively communicates with RMCC (Remote monitoring call center) registered nurse, MD and other health care staff regarding changes in patient's condition or treatment
  • Accurately completes documentation of assessment in a timely manner (immediately)
  • Assists the RMCC registered nurse in assessment of patient's condition and contributes information to aid in planning and evaluating care
  • Considers the physical, psychosocial and emotional needs of the patient
  • Attend compliance training and adhere to the organization standards of conduct, policies and procedures
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide
  • Maintains a confidential record of patient's progress, symptoms, reactions, and treatments performed, interventions and care provided
  • Maintains/ enhances nursing knowledge base through in-services, literature review, mandatory and competency programs
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization
  • Possesses excellent communication skills, both oral and written for utilization during phone calls, documentation and communication with MD and Disease State Management Coordinator
  • Proven ability to facilitate change
  • Provides Remote Monitoring assessment to the patients under the supervision of a registered professional nurse
  • Provides individualized patient teaching and appropriate counseling related to health prevention, health maintenance, medication and disease process under the direction of the RMCC registered nurse
  • Utilizes the RMCC Quality Assurance Metrics to assure compliance on both incoming and outgoing calls

Qualifications

  • One (1) year Medical/Surgical experience in an acute care facility and/or skilled nursing facility
  • Required Minimum of two (2) years medical/surgical experience in an acute care facility and/or skilled nursing facility - Preferred
  • Graduate of an accredited school for practical nursing - Required
  • Strong computer and communication skills highly preferred

Licensure, Certifications, and Clearances:
Must complete licensure examination within FOUR MONTHS of hire or within one year of graduation, whichever comes first. Though temporary permits are valid for one year from the date of graduation, not one year from issue of permit (or until the results of the examination are known at which time it becomes null and void), UPMC requires that GNs take the licensure examination within 4 months of their start date or they will be terminated or demoted from the GN position, at the discretion of the BU.
