Provide management of multidisciplinary home health care teams to ensure the delivery of quality patient care services in the most cost-effective manner available and to confirm compliance with all regulatory agencies based on agency policy, procedures and direction. Work autonomously with little supervision.

  • Attend compliance training and adhere to the organization standards of conduct, policies and procedures
  • Direct, advise and facilitate patient care integration and coordination according to Conditions of Participation, JCAHO standards and the policies and procedures of the home health agency
  • Focus on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction
  • Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide
  • Monitor adherence to the budget and has overall responsibility for appropriate staffing and productivity for the teams
  • Overall responsibility for ensuring timely billing for teams' patients related to PPS and managed care
  • Responsible for ensuring that the teams are generating reports appropriately and following up as indicated
  • Overall responsibility for utilization management activities within the teams, i.e., ADRs, denials, etc
  • Participate in administrative on call coverage for the agency
  • Provide overall direction and control of the multidisciplinary team's personnel
  • Ensure efficient utilization of personnel


  • Graduate of an accredited school of professional nursing required
  • Bachelor's degree preferred
  • 3 years of home health experience required
  • Management experience required
  • Informatics experience preferredĀ 
  • Strong oral and written communication skills preferred

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance
  • Act 34 Criminal Clearance
  • Automotive Insurance
  • Basic Life Support or Cardio Pulmonary Resuscitation
  • Driver's License
  • Older Adult Protective Services Act
  • Registered Nurse
