Description

Purpose:
Provide direct technical nursing care to patients/families within their home setting under the direction of the primary care nurse and in conjunction with the interdisciplinary team.

Responsibilities:
  • Assist the RN and interdisciplinary team in evaluating, planning, and implementing plans of care.
  • Assists the registered nurse/therapist in assessment of patient's condition and contributes information to aid in planning and evaluating care. Considers the physical, psychosocial and emotional needs of the patient.
  • Assume responsibility for assignment and views problems as challenges.
  • Attend compliance training and adhere to the organization standards of conduct, policies and procedures
  • Communicate effectively with patients and families about hospital stay and home care concerns. Instruct and demonstrates care. Discuss any patient or family concerns with RN in charge so that issue can be appropriately addressed.
  • Complies with safety policies and procedures including standard precautions.
  • Constructively receives feedback and direction.
  • Demonstrate awareness of behavior on the efficient functioning of the department.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction
  • Identifies and communicates learning needs to the unit director.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide
  • Maintains a confidential record of patient's progress, symptoms, reactions; treatments performed, interventions and care provided. Completes documentation including visit assessment accurately and promptly on the laptop device or in the written documentation while in the patient's home and within the agency and regulatory standards. Data syncs (transmits) information same day. This information is communicated to the primary service provider and appropriate interdisciplinary team members in a timely and thorough manner.
  • Maintains current CPR certification at Health Care Provider level.
  • Maintains nursing knowledge base through inservices, literature review, mandatory and competency programs
  • Observe and record patient's symptoms, condition, and/or reactions to medications and treatments and reports results to the RN in charge.
  • Participates in project and committee work.
  • Participates in self-review as requested by unit director.
  • Perform assigned work in a timely and productive manner.
  • Performs all other duties as assigned.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of UPMC/Jefferson Regional Home Health, L.P. towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Performs in accordance with the system-wide behaviors and competencies.
  • Prior to performing new or challenging skills, requests assistance and reviews relevant policies and procedures.
  • Provide appropriate, direct patient care. Give injections, takes vital signs, performs basic diagnostic tests, observes patients, dresses wounds and administers medication. Complies with established medication use policies.
  • Provides individualized patient/caregiver teaching and appropriate counseling related to health prevention, health maintenance and disease process under the direction of the registered nurse
  • Provides individualized skilled home health nursing care to patients under the supervision of a registered professional nurse.
  • Requests assistance to help in planning and prioritizing activities as needed.
  • Serves as a preceptor for new staff members.
  • Serves as a resource for staff and other disciplines.
  • Strictly adhere to all policies and practices relative to patient care and hospital-related information.
  • Suggest and supports changes within the department.
  • Takes action to improve knowledge, skills and performance based on feedback or on self-identified developmental needs.

Qualifications

  • Graduate of a Practical Nursing Program approved by the State Board of Nursing required
  • One (1) year Medical/Surgical experience in an acute care facility and/or skilled nursing facility required
  • Minimum of 2 years med/surg experience in an acute care and/or skill nursing facility preferred

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
