Responsibilities:

  • Accept assigned duties in a cooperative manner.
  • Add resident names to optometry and dental clinic lists as requested by resident or responsible party and ordered by attending physician. Coordinate these clinics and related services.
  • Assist in resolving issues perceived by the resident or sponsor/family as a problem.
  • Assist resident and sponsor/family with adjustment to placement.
  • Assist resident and/or sponsor/family in formulating resident goals, and documents goals and Social Service approaches to help resident in meeting goals; documents progress and updates care plan as required.
  • Assist resident as needed in obtaining appropriate clothing and other personal items.
  • Assist resident or responsible party in completing the application for Medical Assistance and copying all documents that are required to accompany application.
  • Assume accountability for information contained in the employee handbook.
  • Attend all assigned in-services.
  • Carry pager to coordinate admissions after hours and on week-ends in rotation with other Social Workers and Admissions Director.
  • Collect social history data, as required, in an organized, legible and accurate format.
  • Come to work as scheduled and consistently demonstrate dependability and punctuality.
  • Complete as required, accurate initial and quarterly assessments, including physical, psychological, social and spiritual factors impacting on a resident.
  • Complete discharge summary for all assigned residents, including CTB, discharges, and transfers out as required.
  • Complete the Social Service portion of the MDS, as required along with corresponding progress note.
  • Consistently work cooperatively and pleasantly with all staff, residents, family members and others.
  • Coordinate and facilitate Family Support Group and makes available educational material for residents, families/sponsors and the general public.
  • Coordinate discharge plan with facility staff, resident, sponsor/family, and appropriate community agencies, and make timely and appropriate referrals. Document discharge planning progress in Social Service progress notes.
  • Coordinate the lost and found program.
  • Develop and maintain good relations with community agencies/individuals which may be resources for the Department.
  • Develop and maintains Department Policies and Procedures.
  • Develop, coordinate, facilitate and/or present in-services on Resident Rights and other subjects pertinent to staff training and within the Social Service field of knowledge.
  • Document on interdisciplinary care plan, as required, anticipated discharge plan, and updates documentation as changes occur.
  • Explain admission documents to residents/sponsors in a comprehensible manner.
  • Explain applicable long-term care regulations relating to Resident Rights, Medicare, and Medical Assistance to resident, sponsor/family, and interdisciplinary team as they impact on the resident.
  • Follow Resident Rights policies at all times.
  • Follow established performance standards and perform duties according to Social Service and Admissions policies and procedures.
  • Invite residents who are capable of participating to attend and participate in care planning meetings.
  • Listen to and counsel resident/family regarding issues related to death, dying, grieving, loss, change, disability, and assertive behavior.
  • Maintain ongoing supportive contacts with resident and resident's sponsor/family, documenting its effect on the residents.
  • Manage Department, maintaining office and supplies conducive to completing duties and meeting public.
  • Observe Infection Control and Universal Precaution policies and procedures.
  • Observe all facility safety policies and procedures.
  • Participate in care planning conferences, utilizing significant psychosocial aspects of resident assessment, and takes a leadership role as assigned by the RNAC.
  • Participate on facility committees as assigned.
  • Provide crisis intervention for unit emergencies.
  • Provide resident with drawer key when requested.
  • Provide the public with information on SMSP, long term care, advance medical directives, and community resources.
  • Represent Department at monthly Family Council meeting
  • Represent Department at monthly Resident Council meeting.
  • Supervise and train Social Service and Department Volunteers and delegates duties appropriately.

Qualifications

A Bachelors degree in Human Services required, Masters preferred. At least two  years experience in health care, preferably long term care.

Licensure, Certifications, and Clearances:
  • Act 34 Criminal Clearance
  • Older Adult Protective Services Act
