Description

Purpose:
Provide Social Work Services to patients and families as part of a comprehensive plan of care. This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance and in accordance with Agency policies, procedures and code of ethics.

Responsibilities:
  • Adhere to Agency and standard principles of infection control.

  • Educate and require adherence to performance improvement processes and initiatives
  • Endeavor to achieve and maintain quality and customer satisfaction in the delivery of services to internal/external customers.
  • Expand knowledge and expertise through educational resources and literature review
  • Maintain and protect customer and Agency confidentiality
  • Maintain personal productivity by demonstrating satisfactory attendance and punctuality
  • Model and mentor effective leadership and management
  • Participate in Agency educational programs, orientation, quality assurance, and quality improvement activities.
  • Participate in program planning when social service needs are involved.
  • Perform quality assurance and utilization review audits and develop reports.
  • Possess Agency equipment required to perform evaluations and implementation of care as outlined in plan of care and procedures.
  • Prepare clinical and progress notes. Provide individual and group counseling services for patients to assist them in coping with their social, emotional, economic and health needs. Evaluate the effectiveness of the social work plan of care according to the patient's need(s) and response(s). Identify variances to established goals/outcomes and revise plan of care in consultation with Social Worker II. Consult and coordinate services with other disciplines and internal/external health care team members or resources. Utilize and provide client/family with appropriate community resources as needed. Advocate for the patient/family in securing resources from health care providers. Assist other team members in understanding the social and emotional factors related to the health problems. Document and communicate observations and evaluations through reports, records and conferences. Communicate and consult with the patient's physician.

  • Provide social work services for patients and their families, using techniques applicable to the social work role. Identify the social, emotional, and economic needs of patients through a continuous evaluation process. Assess psychosocial status. Establish a plan of care in consultation with the physician and the overseeing Social Worker II. Implement the plan of care in accordance with objectives mutually established with the patient and family.
  • Represent and participate in Agency professional and community activities as requested
  • Responsible for discharge of patients. Plan and prepare for discharge on an on-going basis, documenting all coordination activities and arrangements for on-going care. Coordinate referrals to internal/external resources to assure attainment of outcomes. Assure the discharge summary is accurate and complete. Complete discharge instructions for the patient at the time of discharge.
  • Self assess educational needs and initiate plan to maintain and attain general and specialty clinical competencies.
  • Serve on Agency committees as required providing social services input and expertise.

Qualifications

Education: Baccalaureate Degree in social work, psychology, sociology, or other field related to social work required. Experience: Minimum of one year of social work experience in a health care setting. Computerization Skills: Competency in computer skills in a Windows based environment preferred. Travel: Access to transportation in the Agency service area with valid Pennsylvania drivers license and auto liability insurance.

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
