Our Skilled Nursing & Rehabilitation Center is located in Clinton County and is connected to UPMC Susquehanna Lock Haven. We provide high quality care for our residents and exceptional rehabilitation and skilled nursing services for short and long term patients.


In this role you would promote independence of residents through utilization of general recreation resources. You would also provides activity services and assist with department functions through completion of supportive duties.

Responsibilities:
  • Actively practice behaviors associated with SH Service Excellence expectations. Smile at and greet everyone enthusiastically, treat everyone with respect, dignity and C.A.R.E.2. Be an ambassador for UPMC Susquehanna. Provide the best C.A.R.E.2 for our patients no matter what.
  • Assists with the transportation of residents
  • Assume responsibility for personal development and education
  • Contributes to the resident newsletter and other media by taking pictures and providing information as needed
  • Delivers calendars, newsletters, activity books to resident rooms
  • Incorporate leading/evidence-based practice in quality/safety into every day work. Commit to high quality, safe work; encourage others to have similar standards. Identify processes/systems that could lead to inefficient practices, unsafe conditions, errors or adverse events. Avoid shortcuts to ensure quality and safety measures are not circumvented. Encourage patient and family involvement in processes involving patient safety. Speak up with other service partners to ensure high quality and safety. Report serious events/incidents per policy
  • Know and follow all organizational/departmental policies/procedures.
  • Organizes and leads activities in 1:1 or group setting, taking into account the needs and interests of individual residents; modify activities as needed to maximize resident success.
  • Plans and promotes resident activities in advance to provide quality programs.
  • Records resident activity participation daily on the electronic computer documentation system
  • Requests needed supplies in advance to ensure material availability

Qualifications

Education/Qualifications:

  • High School Diploma or equivalent Required.
  • One year Long Term Care or Elderly Population Care experience preferred.
  • Work or volunteer experience providing recreation services to special populations or senior citizens preferred.

Licensure, Certifications, and Clearances:

  • Nurse Aide Certification Preferred
  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Basic Life Support or Cardio Pulmonary Resuscitation
  • Older Adult Protective Services Act
