Responsible for coordinating both the patient care and office operations for the center. Organize all aspects of the program including coordinating support staff, clinical activities to support research and patient care.

Responsibilities:
  • Administer and maintain departmental policies and procedures, and notify staff of additions or modifications, including HIPPA with reference to patient care and research.
  • Assist with educational activities with physicians, hospitals and community, including lectures, seminars and educational materials.
  • Coordinate multidisciplinary clinical conference including case presentation schedule, assimilation of pertinent medical records, and attendance records.
  • Coordinate referrals and services with other health care providers as appropriate.
  • Implement patient care plans and ensure follow up on all aspects of the plan.
  • Participate in clinical and community outreach programs and assist coordinating programs to provide education and enhance consumer awareness.
  • Provide coordination of services and patient care to ensure program goals and objectives are achieved by providing direction to program staff, maintaining efficient operations, and implementing and reviewing quality of care and customer service.
  • Provide evaluation, nursing intervention, individualized patient and family education and supportive counseling.
  • Responsible for following the mandatory reporting procedures for any incident or serious event that did affect or potentially could have affected the clinical care of any patient.
  • Serve as liaison and resource person to community-based general and specialty physician practices to ensure comprehensive services are rendered.
  • Serve as primary recipient (triage) for patient calls and assesses patient needs. Provide general education information and coordinate services or referrals as appropriate.
  • Work collaboratively with other professionals to provide comprehensive patient care and development of new programs and services.

Qualifications

  • Graduate of an accredited School of Nursing. 
  • Minimum of five years of clinical experience. 
  • Pulmonary sleep experience preferred. 
  • Knowledge of patient care, clinical protocols and analytical techniques normally acquired through the completion of a Practical Nurse educational program. 
  • Successful demonstration of the completion of the competencies required by the department/unit. 
  • Knowledge of clinical nursing theory including patient evaluation and education normally acquired through a course of study resulting in a license for practical nursing. 
  • Strong management and clinical abilities for integrating all aspects of the program. 
  • Effective interpersonal skills to communicate with the health care team, patients, families, fellows, interns, students, hospitals and the public. 
  • Knowledge and experience of department support and clinical information systems for the management, update and maintenance of systems; Kronos, SMS, EPIC, MARS, Data Repository and Power chart. 
  • Ability to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. 
  • They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needs as described in the departments policy and procedures.

Licensure, Certifications, and Clearances:
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
  • Act 34 Criminal Clearance
  • Licensed Practical Nurse
