Provide personal care, household management, and companionship services as designated in the customer's plan of care. This will be done in a manner consistent with Department of Health and Office of Long Term Living regulatory requirements.

  • Assist with marketing and promotion of services.
  • Assist with orientation and ongoing staff training as required by the DOH regulations.
  • Be available ongoing, as requested by the Care Supervisor, as a field resource and advisor for employees for care related issues regarding challenging clients.
  • Complete ongoing and annual educational requirements to maintain competency.
  • Conduct on-site supervisory field visits with staff within 30 days of hire and annually thereafter, as requested by the care supervisor.
  • Coordinate caregiving schedules for staff.
  • Function as a mentor to newly hired staff during orientation and ongoing as needs arise.
  • Help to ensure client documentation is compliant with PA Department of Health regulations.
  • Intake and assign customers/clients to direct care staff.
  • Maintain and protect customer, staff, and Agency confidentiality.
  • Participate in on-call rotation.
  • Participate with employee recruitment and selection.
  • Perform all direct customer care in accordance with and without deviation to the established Plan of Care, safety procedures, and within the guidelines of Agency policies and procedures.
  • Perform duties of Care Giver including client assistance with: personal care, household management tasks, light housekeeping, laundry, shopping and errands, pet care, meal preparation, companionship, escorting, and medication reminders.
  • Provide hands on practical training in the field to new employees that shadow them during orientation in order to enhance their personal care skill performance and approach to providing excellent customer service.
  • Regularly attend required staff meetings.
  • Report customer concerns/complaints to the Care Supervisor for immediate follow up and resolution.
  • Report, document, resolve and follow up on client complaints and concerns.
  • Responsible for ensuring accurate and timely documentation of care provided through effectively using the telephony system according to established procedures.
  • Responsible for observing and immediately communicating to the Care Supervisor any significant changes in a customer's condition, suspected cases of abuse or neglect, and safety issues or concerns.
  • Submit documentation within Agency defined timeframes.


High School Diploma or GED preferred. 1 year of related work experience and CNA required or 2 years of related work experience required. Access to transportation in the Agency service area with valid Pennsylvania drivers license and auto liability insurance. No weight restrictions for lifting or moving.
