Are you an experienced Social Worker with a passion for Hospice, and home health care? This is the role for you! The home health/hospice social worker will
provide social work services to authorized Hospice patients and their families. This will be done in a manner consistent with efficiency, cost plan, organize and implement Social work services in accordance with agency policies and procedures.

  • - Assess special needs related to cultural diversity including communication, space, role of family members and caregivers, and special traditions. - Assess special needs related to cultural diversity including communication, space, role of family members and caregivers, and special traditions. - Identify the developmental level of patient, family and caregiver and obstacles to learning or ability to participate in care of patient. - Identify obstacles to compliance and assisting in understanding goals of intervention. - Identify support systems available to reduce stress and facilitate coping with end-of-life care. - Evaluate for long-term care when appropriate and assess ability to accept change in level of care. - Identify family dynamics and communication patterns. - Involve the patient, family and caregiver in the plan of care. - Address patient/family questions and issues. - Identify family members and caregivers for bereavement services, including those who may be high risk.
  • Adhere to Agency and professionally acceptable standards, policies and procedures. Endeavor to achieve and maintain quality and customer satisfaction in the delivery of services to internal and external customers. Demonstrate support of Agency mission and philosophy in delivery of services. Perform other duties and responsibilities as requested.
  • Assess the psychosocial, spiritual, emotional factors, and economic needs related to the terminal illness of Hospice patients, families and caregivers in accordance with established polices and procedures, through a continuous assessment process. - Establish a Plan of Care in consultation with the physician and the overseeing Social Worker II. - Assist the physician and other Interdisciplinary Team members in recognizing and understanding social/mental stress and/or discord that exacerbates the symptoms related to the terminal illness. - Assess the patient, family or caregivers psychosocial status, potential for risk of suicide and/or abuse or neglect. - Assess environmental resources and obstacles to maintain safety. - Assess family and caregiver's ability to function adequately. - Assess need for counseling related to risk assessment for pathological grief.
  • Attend Hospice Interdisciplinary Team meetings on a regular basis. Participate in inservice programs, orientation, continuing education programs, clinical conferences, quality assurance and performance improvement activities. Maintain and protect patient and Agency confidentiality. Promote personal and co-worker safety during work duties. Represent and participate in Agency professional and community activities as requested.
  • Communicate psychosocial information to inpatient facility when level of care is changed. Evaluate the effectiveness of the social work plan of care and make revisions as needed. Develop and participate in the implementation of the bereavement care plan for Hospice patient's family and caregivers. Participate in individual, family, staff, volunteer, and community support groups and educational inservice programs dealing with bereavement. Communicate and document observations, services and evaluations through reports, progress records, and conferences. Prepare clinical and progress notes.
  • Initiate interventions specific to emotional and psychological support to the Hospice patient, their families and caregivers. Identify and utilize appropriate community resources and assess the patient, family and caregiver's ability to access them. - Evaluate patient, family and caregiver response to intervention when referred to community agency and satisfaction of service provided. Evaluate the patient, family and caregiver response to psychosocial interventions.
  • Participate in the development and revision of a plan of care in cooperation and approval of the Hospice Interdisciplinary Team for those needs that can be met through social work skills. Assist other staff to obtain data to facilitate problem identification and development of the plan of care. Provide emotional and psychological support to patients, families and caregivers; addressing patient, family and caregiver questions and issues. Provide social services including short-term individual counseling, crisis intervention, assistance in providing information and preparation of advance directives, funeral planning issues and transfer of responsibilities regarding fiscal, legal and health care decisions. Assist family and caregiver in assessing financial resources when appropriate.
  • Provide social work services under the direction of a physician and in accordance with a plan of care.


  • Bachelors Degree in Social Work required, from an institution accredited by the Council on Social Work education or Bachelors degree in Psychology, Sociology or other field related to Social Work.
  • Minimum of one year related work experience.
  • Competency in computer skills in a Windows based environment.
  • Demonstrate ability to perform computer skills in specialized computer application(s) as needed.

  • Act 34 Criminal Clearance
  • Older Adult Protective Services Act
