Purpose:
Coordinate patient safety activities, including identifying patient safety issues, national trends and appropriate benchmarking with other health care institutions. Oversee patient safety staff.

Responsibilities:
  • Coordinate the development and/or revisions of policies and procedures and the hospital-wide patient safety plan to comply with regulatory agencies ( Joint Commission, PA Department of Health) to ensure safe operations.
  • Establish and maintain effective communication channels with all departments and facilitate information sharing between departments.
  • In collaboration with Vice President Medical Affairs and Chief Nursing officer and designees, oversees systems for reportable Patient Events and Infrastructure Failures according to state regulations.
  • Monitor, trend and analyze data and identifies strategies to reduce and prevent events. Coordinate multi-disciplinary teams as appropriate to implement patient safety strategies.
  • Oversee the investigation for serious events and incidents. Responsible for triaging patient safety occurrences for further action including actions immediately necessary to ensure patient safety, root cause analysis and external reporting.
  • Prepare and present reports of patient safety information through established structures within the organization.
  • Responsible for conducting Intense Analysis (RCA) meetings and ensure implementation of system redesign based on recommendations. Track and monitor follow-up actions for RCA's.
  • Responsible for daily operations of area; assess performance of patient safety staff.
  • Responsible for fostering a culture of safety, proactive risk assessment.

Qualifications

  • Bachelors degree in a health care field is required. 
  • Masters Degree preferred.
  • Minimum of 5 years clinical experience is required.  
  • Previous experience managing complex projects is required. 
  • Previous experience in risk management or process improvement is required. 
  • Basic computer knowledge is required.

Licensure, Certifications, and Clearances:
  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Registered Nurse
