Description

UPMC is looking for a Focus Care Wellness Guide! This is a Full-Time position with up to 40 hours per week. Hours range Monday - Friday between 8:00am - 4:30pm. This position will be responsible for completing monthly outreach calls to assist patients with health and wellness goals such as, diet, exercise, preventive health measures, etc. The ideal candidate will have a background in coaching or nutrition.  

Purpose:

Work with a care team consisting of RN Care Managers, and other wellness guides to perform monthly outreach to a caseload of patients enrolled in the UPMC focused Care program. In addition, they will work with Physicians, Advance Practice Providers, Social Workers, Behavioral Health therapists, Diabetic Educators, practices nurses, medical assistants and office staff in order to help patients manage and improve their chronic conditions in order to reach population health management goals of improving patient access to care, reducing number of emergency visits, and decreasing readmissions.

Responsibilities:
  • Actively promotes patient engagement through wellness guidance and self-management support and promotes patient health literacy through learned motivational interviewing and communication techniques.
  • Collaborates with care team members to reach population health management goals of improving patient access to care, reducing number of emergency visits, and decreasing readmissions
  • Competently employ motivational strategies for encouraging individuals to explore motivation for change and enhance readiness.
  • Create an encounter and properly document each telephonic visit.
  • Define health behaviors or concerns that would potentially improve health if targeted for change.
  • Develop and maintain awareness of and competence in the use of behavior modification skills.
  • Develop and maintain knowledge of UPMC and community health resources. Accurately document member contacts in a timely fashion. Track member progress toward defined goals. Provide timely feedback to primary care physicians about members' progress toward health behavior goals.
  • Develop and maintain knowledge of interventions for obesity, smoking, diabetes, improving compliance to medication and/or medical regimens, and others as indicated.
  • Educate patients to develop self-management skills related to new and chronic conditions utilizing approved UPMC materials at the patients level of understanding.
  • Engage members who have specified risk factors in the collaborative development of a personalized behavior change plan to address the risk factor(s) in conjunction with their primary care physicians, if appropriate.
  • Engage with members in regular contacts over time to assist them in implementing the change plan, tracking progress, and maintaining changes over time.
  • Manage and run daily reports on patient population to actively target patients in need.
  • Participate in population health outreach to patients and assist with gap closure.
  • Perform telephonic outreach to patients on a regular basis to educate, motivate and help facilitate behavior changes in patients with multiple chronic conditions.
  • Send/receive patient medical records in accordance with Health Insurance. Portability and Accountability Act (HIPAA) and other state and federal regulations.
  • Telephonic outreach to patients discharged from the hospital or emergency room to schedule follow up appointments and review medications and treatment plans
  • Use motivational interviewing techniques and patient coaching skills to work with patients to create, evaluate and reach personalized health and wellness goals and identify and overcome barriers related to a healthy lifestyle.

Qualifications

  • High school diploma or equivalent is required.
  • Three years of direct patient care experience required.
  • Must have basic computer skills and knowledge of medical terminology 
  • Preferred experience with Patient Education or as a Health Coach.
  • Must complete UPMC Health Coach Training within first 60 days in role.

Licensure, Certifications, and Clearances:
Must have a current medical certification in one of the following programs: Medical Assistant, LPN, RN, EMT, Health Coach, Dialysis, Nutritionist or other Health Care relevant field

  • Act 31 Clearance
  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Advanced Cardiac Life Support or Basic Life Support or Cardio Pulmonary Resuscitation
