Provide direct technical nursing care to patients/families within their home setting under the direction of the primary care nurse and in conjunction with the interdisciplinary team.

  • Assist the RN and interdisciplinary team in evaluating, planning, and implementing plans of care
  • Assists the registered nurse/therapist in assessment of patient's condition and contributes information to aid in planning and evaluating care
  • Considers the physical, psychosocial and emotional needs of the patient.
  • Assume responsibility for assignment and views problems as challenges.
  • Attend compliance training and adhere to the organization standards of conduct, policies and procedures
  • Communicate effectively with patients and families about hospital stay and home care concerns
  • Instruct and demonstrates care
  • Discuss any patient or family concerns with RN in charge so that issue can be appropriately addressed
  • Complies with safety policies and procedures including standard precautions
  • Constructively receives feedback and direction
  • Demonstrate awareness of behavior on the efficient functioning of the department
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction
  • Identifies and communicates learning needs to the unit director
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide
  • Maintains a confidential record of patient's progress, symptoms, reactions; treatments performed, interventions and care provided
  • Completes documentation including visit assessment accurately and promptly on the laptop device or in the written documentation while in the patient's home and within the agency and regulatory standards
  • Data syncs (transmits) information same day
  • This information is communicated to the primary service provider and appropriate interdisciplinary team members in a timely and thorough manner
  • Maintains current CPR certification at Health Care Provider level.
  • Maintains nursing knowledge base through inservices, literature review, mandatory and competency programs
  • Observe and record patient's symptoms, condition, and/or reactions to medications and treatments and reports results to the RN in charge
  • Participates in project and committee work
  • Participates in self-review as requested by unit director
  • Perform assigned work in a timely and productive manner


  • Graduate of a Practical Nursing Program approved by the State Board of Nursing required
  • One (1) year Medical/Surgical experience in an acute care facility and/or skilled nursing facility required
  • Minimum of 2 years med/surg experience in an acute care and/or skill nursing facility preferred

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance
  • Act 34 Criminal Clearance
  • Automotive Insurance
  • Cardio Pulmonary Resuscitation
  • Driver's License
  • Licensed Practical Nurse
  • Older Adult Protective Services Act
