Purpose:
Consistent with the philosophy of the Nursing Department, the LPN is expected to deliver quality care to assigned residents under the direction of a professional registered nurse in accordance with Avalon Springs Nursing Centers policies, procedures, state and federal regulations. The Licensed Nurse directs the nursing assistant, and utilizes the nursing process so that the physical, medical and emotional needs of the resident/patient are met.

Responsibilities:

  • Administers medication and treatments accurately, observing resident response, as evidenced by documentation in the medical record and lack of negative outcomes. Monitors I.V. sites and administers I.V. meds through peripheral lines if LPN has basic I.V. certification.
  • Anticipates and takes action to maintain a safe environment. Incorporates safety/risk management standards into daily work practices. Uses all safety equipment provided appropriately. Reports faulty equipment/problems immediately.
  • Assists in the orientation/training of new nursing personnel.
  • Complies with the facilitys infection control policies and procedures including Body Substance Isolation (BSI) and the Exposure Control Plan that reflects OSHA Bloodborne Pathogens Standard to ensure a safe working environment for self and others.
  • Consistently works in a positive and cooperative manner and assists other nursing personnel by: - Consistently demonstrates ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary. - Provides emotional support to residents and families as needed. - Reports any allegations of resident abuse of any type to RN immediately and take corrective action. - Effectively responds to emergency situations in the absence of the RN. - Recognizes verbally or physically aggressive resident behavior, responds appropriately and educates other staff/family on safe procedures. - Responds to resident call lights and intercom and door alarms in an appropriate and timely manner. - Minimizes non-productive times with activities that will prepare to meet the future needs of the facility (education, organizing, housekeeping, assisting others). - Organizes job functions and work areas to effectively complete varied assignments within established time frames. - Consistently demonstrate ability to take the initiative to make decision/choices without direct supervision. - Seeks out opportunities to help rather than waiting to be asked.
  • Demonstrates knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals.
  • Demonstrates sound judgment by taking appropriate actions regarding questionable findings or concerns. - Investigates and follows through on unusual orders or requests for service or information. - Follows proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur. - Records/reports the need for service maintenance or repair of equipment and remove any faulty equipment from service. Uses all safety equipment provided.
  • Documents all aspects of residents care as accurately, concisely, and promptly as per state and federal requirements. - Contributes to the development of the resident/significant other plan of care based on the identified resident problems/needs. - Assists in the evaluation of the effectiveness of the nursing care administered relative to the resident/significant other identified problems/needs.
  • Maintains and coordinates communication among resident/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback. - Assists in assuring that effective communication exists with resident/ significant other as reflected in the plan of care. - Gives punctual change of shift report consisting of pertinent and relevant information for assigned residents. - Effectively communicates residents changing needs and plan of care among staff members. - Reports residents needs/problems and observations regarding residents condition to the registered nurse and physician. - Attends physician rounds on assigned residents and communicates observations - regarding residents condition. - Reports resident abuse immediately to supervisor; takes corrective action.
  • Maintains confidentiality of resident information at all times.
  • Maintains optimum nursing practice through participation in internal and external training enrichment programs geared toward geriatric residents and residents of various ages.
  • Observes the activities of all nursing assistants assigned to area of responsibility. Guides, teaches and develops nurse aides to ensure safe, quality care for geriatric residents and other residents of various ages. Monitors Certified Nurse Aids activity and reports non-compliant acts to RN Supervisor immediately for correction.
  • Participates in Performance Improvement Committee, Safety Committee, etc., as needed.
  • Participates in staff development programs by: - Attending scheduled staff meetings and mandatory in-services. - Assisting with staff meetings and mandatory in-services. - Obtaining information discussed and incorporating the new information into practice.
  • Provides resident/significant other health teaching as delegated by and observed by the RN.
  • Reports any suspected or known resident abuse to Director of Nursing or Administrator immediately.
  • Reports for work as scheduled and consistently demonstrates dependability and punctuality.
  • Safely administers quality skilled care in compliance with nursing policies and procedures, physician orders, and to provide external regulatory requirements for established plan of care.

Qualifications

Current Licensure as Licensed Practical Nursing in Pennsylvania or temporary practice permit required. Minimum one-year experience in long term care or other clinical nursing experience preferred.

  • Act 34 Criminal Clearance
  • Basic Life Support or Cardio Pulmonary Resuscitation
  • Licensed Practical Nurse
  • Older Adult Protective Services Act
