Community HealthChoices (CHC) is Pennsylvania's managed care long term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid.


The Community Workforce Partner is responsible for developing, implementing, maintaining, and monitoring the CHC Workforce Development strategy to enhance the experience of all Direct Care Workers (DCW) through innovative trainings, information sharing, and technological updates. The Community Workforce Partner will drive DCW job satisfaction, retention, training, and other functions aimed at making them a crucial part of the members of the treatment team.


  • Conduct targeted outreach to local organizations to recruit young men and women of diverse, unrepresented background to train and develop them into the next generation of Direct Care Workers
  • Create awareness and programs for Direct Care Workers and other staff to access development training through partnership such as Institute on Aging, Disability resource Center.
  • Create best practices toolkits
  • Develop Direct Service Workers curricular that integrates technical skills development, life and soft skills capacity building and experiential learning
  • Develop and maintain ongoing relationship with Direct Care Workers network and provide best practice to the agencies and workers
  • Facilitate relationship that yield high quality outcomes between Direct Care Workers Agencies and CHC members.
  • Lead health career awareness by delivering organized training to Direct Care Workers and agencies
  • Organize and standardize trainings among the Direct Care Workers in UPMC CHC provider networks and ultimately statewide
  • Provide professional development and training programs addressing the complex needs for the CHC Participants
  • Represent and advocate for Direct Care Workers workforce coalitions to elevate career trajectory
  • Serve as Disability Consultant to develop and update disability awareness trainings and programs for staff, in partnership with the Disability Resource Center address issues arising from disability related issues and accommodations.


  • Bachelor's Degree in business, Management, or related field required, with a minimum of 3 years of work experience.
  • Proven ability to manage relationships with community partners and internal partners.
  • Proven record of development and implementation of community workforce programs.
  • Demonstrate credibility across public, private and community sectors.
  • Persuasive communicator both written and oral.
  • Experience implementing disability awareness initiatives and advocating for people with disabilities the aging population preferred
