Purpose:

University of Pittsburgh Physicians is currently hiring for a regular Full Time Licensed Practical Nurse, OP (LPN) to support the outpatient General Internal Medicine team located at UPMC Montifiore/Presbyterian Hospital located in Oakland/Pittsburgh, PA.

This position will work a variable Monday through Friday daylight schedule and will provide and help to lead quality initiatives for the department.

This position will focus on closing care gaps for our shared savings patients, communicate with the physicians in regards to their health plan patients who need to be seen, keep up to date with programmatic changes to our plan that may present opportunities for increased savings, maintain and analyze reports uploaded to the Box from the Health Plan, and manipulate data and spreadsheets to create reports for the division.

Provides professional nursing care to office patients under supervision of patient care providers.

Responsibilities:
  • Phone work with patients gathering date and providing follow-up
  • Comply with all policies and procedures.
  • Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA, PA Department of Health).
  • Document all activities/interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record.
  • Maintain strict confidentiality related to medical records and other data.
  • Promote wellness by providing patient education materials, communicating physician advice/instructions.

Qualifications

  • Graduate of an approved LPN school of nursing. 
  • One year clinical experience preferred
  • EMR/Triage experience preferred
  • Excel experience preferred

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 34 Criminal Clearance
  • Advanced Cardiac Life Support or Basic Life Support or Cardio Pulmonary Resuscitation
  • Licensed Practical Nurse
