The hours for this position will rotate daylight, evenings, nights, weekends, and holidays.

Purpose:
Perform daily activities of medication order entry and IV admixture preparation and checking. Clarify invalid and unclear orders with the prescriber including variances from defined standards and interventions such as drug interactions and dosage.

Responsibilities:
  • Maintain involvement in drug interventions and monitor as defined by the Department including the review of dosing regimens for selected agents, recommendation for monitoring patients, and suggestion for dosage modification when necessary.
  • Participate in the design and implementation of drug use evaluations, and is involved in data collection and clinical intervention when appropriate.
  • Perform the drug dispensing and IV preparation activities and ensures the efficient and accurate delivery of medications to patient care areas.
  • Retrieve drug information from Micromedex, standard texts, and other sources of information and applies it to general patient care questions.

Qualifications

B.S. degree or Pharmacy degree from an accredited School of Pharmacy. ACPE accredited continuing education program.  
Hospital pharmacy experience is preferred. 
Ability to obtain and interpret information in terms of the specific needs of the patient served. 
Knowledge of drug dispensing and IV preparation policies and procedures. Understand the range of treatment needed by the patients served.

Licensure, Certifications, and Clearances:
  • Act 34 Criminal Clearance
  • Pharmacist
