Haven Skilled Rehabilitation and Nursing is seeking a Director of Nursing for their 90-bed skilled rehabilitation unit that includes 56 beds for long-term/rehabilitation unit and 34 beds for the Memory Care Unit. 

Haven Skilled Nursing & Rehabilitation Center is located in Clinton County and is only a short walking distance from Lock Haven University and is connected to UPMC Susquehanna Lock Haven Hospital. We provide high quality care for our residents and exceptional rehabilitation and skilled nursing services for short and long term patients. 


The DON would be responsible for the 24 hour operation of the Haven Skilled Rehabilitation and Nursing facility. The position would be for day shift Monday-Friday but does require on call availability for occasional work on evenings, weekends, depending on the needs of the building. 


Purpose:

Responsible for the 24-hour operation of the Haven Skilled Rehabilitation and Nursing facility. Has authority, responsibility and accountability for the function and activities of the nursing staff. Plans, directs and reviews the work of all nursing personnel in the care and treatment of residents.


Responsibilities:

  • Assumes responsibility for the timely completion of all legal and regulatory requirements of direct reports.
  • Develops and communicates Susquehanna Health (SH) nursing philosophy, trends and best practices to nursing leadership and staff on a monthly basis.
  • Develops nursing standards of care to ensure residents are accorded quality care in conformity with the mission, vision and values of SH.
  • Develops the staffing plan with appropriate numbers and levels of nursing personnel based on budget and resident acuity. 
  • Develops nursing leadership succession plan and mentors staff for personal and professional growth.
  • Develops, reviews and implements all resident care nursing policies within SH standards of care and CMS guidelines.
  • Ensures effective mechanism for supervision and appreciation review of nursing personal, including personal development plans for each staff member.
  • Ensures skilled nursing is on the cutting edge of best practice care delivery and services.

  • Patient/Customer Experience Orientation and Patient Safety Designs/utilizes the right tools to capture customer satisfaction and evaluate concerns. Creates and promotes a work team environment where focus is routinely placed on achieving the highest level of patient/customer satisfaction. 
  • Hardwires evidence-based best practices to enhance the customer/patient experience. Has an understanding of customer centric service metrics and implements plan to achieve highest levels of patient/customer experience. Understands that patient safety is everyone's responsibility. Speaks up in a non-confrontational and educational manner when seeing a safety breech; develops or participates in a project or action plan to reduce risk or mitigate harm for patients, as appropriate.
  • Prepares and maintains systems to monitor nursing care delivery to meet CMS and Department of Health standards of care including Performance Improvement dashboard of nursing care services.
  • Relationship Management and Effective Communication Has emotional intelligence that enables effective communicate on an interpersonal level, building strong personal relationships. Understands others' emotions while managing their own emotional responses during difficult conversations. 
  • Demonstrates effective interpersonal relationships with all service partners, including physicians and volunteers.

  • Works with the Medical Director to develop new programs and services in long-term care nursing delivery.

Qualifications

Bachelors degree in Nursing required. Masters degree in Healthcare preferred. Three years experience in Long Term Care nursing or five years experience as a Registered Nurse required. Two years supervisory experience in healthcare required.

  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Basic Life Support
  • Registered Nurse
