Looking to take your nursing skills to the next level? UPMC Home Health is hiring a Clinical Coordinator/Team leader! 

 

As a Clinical Coordinator/Team Leader, you will supervise, guide, and monitor activities of the multidisciplinary team to effect quality patient care. This is done through accurate documentation, ongoing education and coordination of the team's operations.

 

Our team of home health nurses at UPMC live and work in the communities they serve, and they provide specialized care to patients of all ages. Our areas of care include pediatrics, cardiopulmonary, ortho/rehab, ob/gyn, wound care, bariatrics, diabetes, oncology, behavioral health, and more! Responsibilities:

  • Analytical ability necessary to address and improve clinical issues and assist with complex problem solving
  • Leadership ability to maintain standards of performance within a national context for professional nursing practice and comply with administrative and regulatory requirements
  • Superior interpersonal and communication skills sufficient to effectively create and maintain a positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families in both written and verbal communication
  • Builds and mends relationships among colleagues within the framework of a healthy workplace
  • Role models keen cultural awareness including preventing and addressing horizontal violence and impairment
  • Maintains a conscious balance between work and personal life through prioritization and delegation of responsibilities
  • Extends trust by acknowledging the contributions of others; listens first, creates transparency in communications, confronts reality, and clarifies expectations
  • Demonstrates good judgment and analytical ability to oversee and coordinate patient care in collaboration with the physician and the multidisciplinary health care team
  • Uses critical thinking skills to effectively advise and direct health care providers in their delivery of patient care while maximizing the efficient use of human and financial resources to achieve productivity standards.
  • Responsive to all departments daily; accountable to establish professional and respectful interpersonal relationships
  • Facilitates teamwork among all departments involved in the patient care process
  • Seeks opportunities to share expertise with other members of the healthcare team within and beyond the clinical unit/department
  • Regularly seeks feedback on performance and applies continuous professional improvement
  • Identifies opportunities for clinical quality as well as workplace improvement


Qualifications

  • BSN preferred
  • Minimum of 2 years healthcare experience - required
  • Minimum of 1 year home health (or hospice) experience - preferred
  • Minimum of 1 years progressive leadership experience with relevant clinical experience - preferred
Licensure, Certifications, and Clearances:
Current licensure as a professional nurse in PA_-Required Current Certification in basic CPR- Required Current driver's license in state of residence- Required Current auto insurance in state of residence- Required Act 33/34 Clearances- Required Act 73 required for Pediatric/OB caregivers- Required

  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Cardio Pulmonary Resuscitation
  • Driver's License
  • Older Adult Protective Services Act
  • Registered Nurse

Salary Range: $28.37 to $47.88
