Description

Purpose:
The Social Worker - BSW will assess and provide for the medical-psycho-social needs of Family Health Center patients. The Social Worker will, plan, coordinate and implement social work services for patients and their families through referrals to local agencies and available community partners and resources.  The incumbent will act as the primary liaison between the Family Health Center and the community agencies.

 

 Responsibilities:


  • Assess the psycho social needs of patients and provide counseling and support.
  • Provide case management services to patients.
  • Collaborate with the healthcare team, patients and families and relevant community resources to develop, document and implement and individualized plan of care.
  • Complete a psychosocial assessment that includes social, economic, cultural, age-related and behavioral factors, history of child, elder or domestic abuse, prior psychiatric or substance abuse treatment, family and social support systems and available resources as appropriate to the situation.
  • Provide crisis intervention services to patients and their families.
  • Demonstrate knowledge of reimbursement systems; ability to educate patients and families regarding services and ability to effectively advocate on behalf of the patient. 
  • Demonstrate appropriate professional practice, maintaining respect for confidentiality and freedom of choice as outlined by the Code of Ethics by the National Association of Social Workers.
  • Demonstrate the ability to identify, assess, counsel and intervene with medically and psychosocially complex patients. 
  • Provide resource-specific information to patients and families, initiating contact with appropriate resources and facilitating the patient / family's ability to accept referrals to external social agencies and services. 
  • Maintain knowledge of federal, state and local assistance programs and community resources.

  • Document social work contacts and interventions in the electronic medical record in a timely, accurate manner. Manage patient registries for target populations.

  • Provide visits to patients’ home and/or community setting to help coordinate additional services for hard to reach patients.

  • Participate in case conferences for targeted patients.

  • Foster linkage to, and retention-in-care of target populations.

  • Attend trainings, meetings and in-services as required.

  • Perform other duties as assigned.

     



Qualifications

  • Bachelor's degree in Social Work required from a school accredited by the Council on Social Work Education.
  • Desire to work with an underserved population.
  • Good communication and interpersonal and organizational skills include ability to engage patients, motivational interviewing, helping patients to form self-directed goals, connections/relationships to community agencies.
  • Comfortable working with prenatal through geriatrics.
  • Ability to work on a  multidisciplinary team, good computer skills, flexibility.
  • Case management experience helpful.
  • Comfortable in a face paced environment.


Licensure, Certifications, and Clearances:

  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance

Salary Range: $15.70 to $25.35 / hour
