Description

The Patient Education Project Manager has the responsibility for management of patient education projects and patient engagement programs and applies expert knowledge and skills to provide the educational support needed to achieve quality patient care outcomes. He/she works with senior leadership, the Wolff Center, clinical operations and directly with patients to improve and implement educational processes with minimal impact on workflows. Transformational leadership contributor to aggressively change the way patients are engaged through education tools.

Responsibilities:
  • Analyze and recommend improvements to workflow processes to support integration of shared decision-making initiatives.
  • Analyze patient education data analytics to identify and implement opportunities for improvement.
  • Anticipate key business and project issues, assess alternatives, and take appropriate measures to resolve in a professional, tactful manner, implementing comprehensive recommendations to achieve desired outcome.
  • Assist the Director, Patient Education and Shared Decision Making with patient education initiatives.
  • Assume responsibility for complex assignments, applying creative and innovative strategies in problem solving to achieve desired goals.
  • Build follow-up mechanism/maintenance plan into project deliverables to ensure that changes to be implemented will be sustainable. Ensure business value has been delivered.
  • Lead initiatives to create a Health Literate organization.
  • Manage community education class registration system.
  • Participate in hospital and community committees, task forces and training programs.
  • Partner with clinical operations teams to develop and refine implementations of patient education/engagement tools leveraging resources including Healthwise/EMMI/Xealth/other.
  • Promote use of innovative digital strategies to strengthen patient engagement.
  • Provide project management and operational oversight to enterprise patient education projects.
  • Serve as an enterprise resource for staff relative to patient education and shared decision making.
  • Serve as internal consultant to recommend best practices, supporting continuous improvement in patient education.

Qualifications

  • Bachelors Degree in Nursing required or an equivalent degree in a related clinical field required.
  • Masters in Education or advanced degree in related field is preferred (extensive related experience will be considered).
  • 5 years of experience in Education, Nursing, or other clinical field required.
  • Excellent computer skills required, including Excel, PowerPoint and eRecord applications.
  • Strong project management, problem solving, organizational, and communication skills (oral and written) are required.
  • Ability to perform planning for large initiatives/projects, managing all tasks required to provide services to meet client and business needs within budget and targeted deadlines.

Licensure, Certifications, and Clearances:
  • Current Pennsylvania licensure or Registered Professional Nurse license strongly preferred.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component required within 30 days of hire
