Description

Purpose:
Manage the provision of quality home infusion nursing services from the point of referral, coordination of care and clinical nursing services.

Responsibilities:
  • Communicate on a regular basis with General Manager and VP of Professional Practice.
  • Complete performance evaluations to assess staff competency and proficiency of staff.
  • Conduct regularly scheduled staff meetings.
  • Demonstrate a self directed and disciplined approach to job.
  • Demonstrate ability to create a positive work environment.
  • Demonstrate ability to remain calm under pressure.
  • Develop and maintain an effective level of communication with staff.
  • Encourage open communication and verbalization of opinions among staff.
  • Ensure appropriate scheduling and assigning of patient care.
  • Ensure appropriate systems are in place for monitoring quality of patient care documentation submitted by clinical staff.
  • Ensure department complies with applicable accrediting body standards, Medicare Conditions of Participation, and Home Health licensing requirements as indicated.
  • Ensure home health staff are aware and have input into center's issues and changes.
  • Ensure implementation of policies and procedures and appropriate patient care practices.
  • Ensure intake process is completed appropriately and timely.
  • Establish and implement processes to assure smooth functioning of day-to-day patient care services and integration/coordination with all disciplines.
  • Establish and implement processes to ensure clinical staff submits all required documentation timely and appropriately.
  • Establish and implement processes to ensure timely and accurate billing of home care services.
  • Exhibit an attitude, which promotes harmony and goodwill in the workplace.
  • Handle client complaints in a timely manner.
  • Keep abreast of local and State laws affecting nursing practice.
  • Maintain confidentiality of patient information.
  • Maintain knowledge of and teaches staff in company policies, procedures and community resources.
  • Maintain professional and technical job knowledge through appropriate resources.
  • Monitor and control departmental expenses.
  • Oversee and coordinate the overall provision of home infusion nursing services.
  • Participate in Chartwell PA's Performance Improvement Program.
  • Participate in annual department budgeting process.
  • Plan and organize work effectively.
  • Prioritize responsibilities and workload.
  • Promote Chartwell culture by providing quality, comprehensive services to all customers through a team approach.
  • Provide and coordinate ongoing clinical learning experiences and in-services for patient care staff to assure staff maintains clinical competence.
  • Provide direct patient care as needed.
  • Provide for orientation for new staff members.
  • Provide ongoing clear and measurable performance expectations for all staff levels to ensure adherence to all company philosophies, policies and regulations.
  • Provide regular feedback to staff regarding their performance.
  • Recruit and hire a qualified clinical and patient care staff.
  • Support sales staff efforts by providing educational programs and participating in team selling.
  • Use appropriate structured interview techniques and selection criteria.

Qualifications

  • B.S. degree in Nursing preferred. 
  • Graduate of accredited diploma or AD program may be considered with demonstrated evidence of professional growth and development and as permitted by state regulation. 
  • 2 + years of nursing supervision / management experience required.
  • 5 years nursing experience.
  • 2 years of homecare or home infusion experience preferred. 
  • Excellent interpersonal skills, written and verbal communication and organizational skills.

Licensure, Certifications, and Clearances:
  • Act 34 Criminal Clearance
  • Registered Nurse

Salary Range: $29.22 to $49.32 / hour
