Description

The Global care coordinator is the main point of contact for international patients in an adult patient setting and will be dedicated to:


  • Locations: UPMC Falk Clinic and UPMC Presbyterian Hospital
  • The Global care Coordinator is the main point of contact for adult international patients coming to UPMC Facilities.
  • Expedite the identification, coordination, and implementation of the patient's plan of care with the physician and finance team.
  • Evaluates requests for inpatient admission/transfers and outpatient appointments in conjunction with registration, admitting, and an hospitality services.
  • Determines proper scheduling requirements and/or limitations to ensure customer satisfaction, patient flow, and utilization of service areas
  • Facilitates communication between physicians, hospitality, finance, and other healthcare entities.
  • Works closely with clinical team members to ensure patient progression along the continuum of care to promote efficient and quality patient outcomes.   


The Global Care Coordinator provides an interdisciplinary approach to medical management, resource utilization, and discharge planning while promoting quality outcomes and customer satisfaction to each global care patient in a cost effective manner. The Global Care Coordinator serves as an advocate of the patient, physician, health care provider, reimbursement source, and the health system. There is ongoing collaboration with the physician, inpatient Care Manager and Social Worker, primary nurse care coordinator, home care coordinator, finance and other members of the multidisciplinary team to individualize the discharge plan, identify barriers, decrease resource utilization, thus achieving appropriate patient outcomes as well as maximizing financial reimbursement.

Responsibilities:

 

  • Collaborate with finance and clinical team to determine applicable level of care related to both inpatient and outpatient services.
  • Work closely with clinical team members to ensure patient's progression along the continuum of care to promote efficient and quality patient outcomes.
  • Monitor appropriateness of admissions, level of care and continued stays, and documents findings based on standards.
  • Evaluate issues, trends and barriers. Prepares reports related to utilization of services, patient placement decisions and recommend process improvements to VP and Director of Global Care.
  • Coordinate and collaborate with finance and embassy as applicable related to ongoing treatment plan, discharge plan and post acute services.
  • Serves as a liaison to members of the clinical team, finance, hospitality regarding requirements and reimbursement expectations in regards to patient scheduling, patient placement, and pre-admission testing activities.
  • Expedite the identification, coordination and implementation of the patient's plan of care with the physician and finance team.
  • Collaborate with the post-acute care providers to ensure transition from acute to post acute care or outpatient care.
  • Facilitates communication between physicians, hospitality, finance and other healthcare entities.
  • Coordinate applicable outpatient follow up in conjunction with physician, patient, family and outpatient entities to ensure patients clinical care needs are managed.
  • Evaluates requests for inpatient admission/transfers and outpatient appointments in conjunction with registration, admitting, and hospitality services. Determines proper scheduling requirements and/or limitations to ensure customer satisfaction, patient flow, and utilization of service areas.
  • Collaborate with Patient Business Services and finance to ensure optimal reimbursement.
  • Collaborate with hospitality services to ensure optimal patient and family experience.
  • Assists in departmental marketing efforts and the education of physicians, physician office staff, and members of the clinical team related to quality improvement and utilization principles.
  • Comply with rules and regulations set forth by the UPMC financial department as well as applicable embassy contacts.
  • Work in conjunction with the inpatient Care Manager and Social Worker to ensure that the discharge planning has been initiated on admission and ensure discharge planning documentation on an ongoing basis. Identify barriers to discharge, plan as appropriate, and identify treatment options in collaboration with physician, finance, and clinical team.
  • Promotes patient safety and is a patient advocate.

 

Qualifications

  • RN license required.
  • Five (5) years of clinical experience required.
  • BSN or related Bachelors degree required.
  • Previous case management, care coordination, customer service experience preferred.
  • Knowledge of healthcare financial and payor issues preferred. 
  •  Excellent customer service, time management and organizational skills. 
  • Strong communication skills to effectively interact within all levels of the organization and with all care team members. 
  •  Strong critical thinking and analytical skills.

    Licensure, Certifications, and Clearances:
    Currently licensed as professional nurse in the Commonwealth of PA.
