Description

Oversee all aspects of the admission process for both internal and external admissions for thecomprehensive inpatient rehabilitation program. Gather information about potential patients and complete assessments for admission in collaboration with the rehabilitation physicians. Initiate and complete required forms. Responsible for referral development and marketing.

Responsibilities:

  • Assist as requested with performing medical record chart audits to assure compliance with regulatory guidelines.
  • Educate patients and significant others, referral sources, staff and physicians of the benefits of the programs, services and admission requirement for comprehensive Inpatient Rehab
  • Participate in and/or serve as team leader of committees that address Commission on Accreditation of Rehabilitation Facilities (CARF), Joint Commission and other rehab standards.
  • Know and follow all organizational/departmental policies/procedures.
  • Coordinate the Rehab admission process, adhering to Medicare regulations and other third-party payor requirements and prepare, submit, and review monthly and quarterly referral, admission, and denial statistics.
  • Responsible for marketing/sales and referral development for Inpatient Rehab admissions.
  • Communicate the status of daily admissions to all involved staff and referring facilities.
  • Assume responsibility for personal development and education.
  • Assist with accurate completion of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) form.
  • Commit to high quality, safe work; encourage others to have similar standards. Identify processes/systems that could lead to inefficient practices, unsafe conditions, errors or adverse events. Avoid shortcuts to ensure quality and safety measures are not circumvented. Encourage patient and family involvement in processes involving patient safety. Understand that patient safety is everyone's responsibility. Assume responsibility for personal education and performance related to patient safety; report unsafe conditions or events immediately; speak up in a non confrontational and educational manner when seeing a safety breech; develop or participate in a project or action plan to reduce risk or mitigate harm for patients, as appropriate.
  • Facilitate approval for inpatient comprehensive Rehab admissions by performing on-site and telephone pre-admission screenings and clinical assessments.
  • Negotiate admission approval and rehab rates with third-party payors.
  • Assist the Rehab administrative team in formulating and communicating program mission, goals, objectives, and action plans.

Qualifications

Graduate of an approved and licensed school for practical nursing or graduate of a NLN accredited school of nursing or Bachelors Degree in Social Work or in an Allied Health Profession whose practice act permits patient assessment required.

Three months experience in a healthcare setting required.

Licensure, Certifications, and Clearances:
Clinical licensure in the professional field in the Commonwealth of Pennsylvania required as appropriate. Functional Independence Measure (FIMS) certification Required within 90 days

  • Licensed Practical Nurse (LPN) OR Licensed Social Worker (LSW) OR Registered Nurse (RN)
