As a Clinical Coordinator/Team Leader, you will supervise, guide, and monitor activities of the multidisciplinary team to effect quality patient care. This is done through accurate documentation, ongoing education and coordination of the team's operations. This position work Monday-Friday, daylight hours with a weekend rotation of every 3 weeks. Holidays and on-call coverage (remote) is also rotated.

  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide
  • Oversees activities of the team including assignment of new patients to multidisciplinary staff
  • Coordination of interdisciplinary services to patients, coordination and scheduling of visits to ensure that productivity is achieved
  • Ensures that orders for services and treatment are obtained by the Case Manager prior to delivery of care
  • Provides discipline specific direct patient care as scheduling requires
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization
  • Proven ability to facilitate change


  • BSN preferred
  • Minimum of 2 years healthcare experience- required
  • Minimum of 1-year home health (or hospice) experience- preferred
  • Minimum of 1 years progressive leadership experience with relevant clinical experience preferred

Licensure, Certifications, and Clearances:
  • Current licensure as a professional nurse in PA-Required
  • Current Certification in basic CPR- Required
  • Current driver's license in state of residence- Required
  • Current auto insurance in state of residence- Required
  • Act 73 required for Pediatric/OB caregivers- Required
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Registered Nurse (RN)
