Responsible for timely and accurate processing of clinical documentation, reports and service charges. Responsible for monitoring and tracking of required staff corrections to ensure documentation and reimbursement compliance. Responsible for referral processing and authorization assistance.

  • Monitors compliance reports and makes corrections based on information contained in the reports including investigating why patient electronic signature is outside of patient visit date. Work with clinician to update.
  • Review face-to-face documents to ensure full completion, route errors or incomplete forms to correct clinician for follow-up.
  • Monitoring, tracking and verification of all clinical documentation corrections related to start of care and resumption of care.
  • Accurately data enters paper documentation.
  • Identifies, develops and participate in process improvement opportunities within the home health agency that will enhance the quality of the services we provide.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Monitoring, tracking and verification of clinical 485/Plan of Care, verbal physician orders, Oasis assessments.
  • Accurately corrects, verifies and enters referrals via Home Care Automation system into the Horizon Home Care system.
  • Work closely with UM reviewers when corrections are needed in Oasis or verbal orders. Communicate with clinician, verify corrections are made.
  • Provides on-going reports to clinical operations for management/operational purposes.
  • Provides primary data entry assistance to authorization professionals and other authorization tasks as designated.
  • Daily Activity Record service charge tracking/validation/generation to ensure accurate billing. Run batch verifications to ensure there are orders for every visit. Notify clinical staff when there are errors. Monitor visits made under 20 mins. for billing purposes.
  • Work with various contractor systems to retrieve patient records, visit notes, daily activity tracker from their electronic systems. Send information to medical records.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.


  • High School diploma or GED required.
  • 1 year data entry experience required.
  • Completion of medical terminology course preferred.
  • Completion of post high school program preferred.
