Provide direct technical nursing care to patients/families within their home setting under the direction of the primary care nurse and in conjunction with the interdisciplinary team


  • Assist the RN and interdisciplinary team in evaluating, planning, and implementing plans of care
  • Provide appropriate, direct patient care
  • Give injections, takes vital signs, performs basic diagnostic tests, observes patients, dresses wounds and administers medication.  Complies with established medication use policies
  • Observe and record patient's symptoms, condition, and/or reactions to medications and treatments and reports results to the RN in charge
  • Communicate effectively with patients and families about hospital stay and home care concerns
  • Discuss any patient or family concerns with RN in charge so that issue can be appropriately addressed
  • Perform assigned work in a timely and productive manner
  • Strictly adhere to all policies and practices relative to patient care and hospital-related information
  • Complies with safety policies and procedures including standard precautions
  • Performs in accordance with the system-wide behaviors and competencies
  • Provides individualized skilled home health nursing care to patients under the supervision of a registered professional nurse
  • Completes documentation including visit assessment accurately and promptly on the laptop device or in the written documentation while in the patient's home and within the agency and regulatory standards
  • Assists the registered nurse/therapist in assessment of patient's condition and contributes information to aid in planning and evaluating care
  • Considers the physical, psychosocial and emotional needs of the patient
  • Provides individualized patient/caregiver teaching and appropriate counseling related to health prevention, health maintenance and disease process under the direction of the registered nurse


  • Graduate of a Practical Nursing Program approved by the State Board of Nursing required
  • One (1) year Medical/Surgical experience in an acute care facility and/or skilled nursing facility required
  • Minimum of 2 years med/surg experience in an acute care and/or skill nursing facility preferred

Licensure, Certifications, and Clearances:

  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Licensed Practical Nurse (LPN)
  • Act 33
  • Act 34
