Description
Purpose:Provide direct technical nursing care to patients/families within their home setting under the direction of the primary care nurse and in conjunction with the interdisciplinary team
- Assist the RN and interdisciplinary team in evaluating, planning, and implementing plans of care
- Provide appropriate, direct patient care
- Give injections, takes vital signs, performs basic diagnostic tests, observes patients, dresses wounds and administers medication. Complies with established medication use policies
- Observe and record patient's symptoms, condition, and/or reactions to medications and treatments and reports results to the RN in charge
- Communicate effectively with patients and families about hospital stay and home care concerns
- Discuss any patient or family concerns with RN in charge so that issue can be appropriately addressed
- Perform assigned work in a timely and productive manner
- Strictly adhere to all policies and practices relative to patient care and hospital-related information
- Complies with safety policies and procedures including standard precautions
- Performs in accordance with the system-wide behaviors and competencies
- Provides individualized skilled home health nursing care to patients under the supervision of a registered professional nurse
- Completes documentation including visit assessment accurately and promptly on the laptop device or in the written documentation while in the patient's home and within the agency and regulatory standards
- Assists the registered nurse/therapist in assessment of patient's condition and contributes information to aid in planning and evaluating care
- Considers the physical, psychosocial and emotional needs of the patient
- Provides individualized patient/caregiver teaching and appropriate counseling related to health prevention, health maintenance and disease process under the direction of the registered nurse
Qualifications
- Graduate of a Practical Nursing Program approved by the State Board of Nursing required
- One (1) year Medical/Surgical experience in an acute care facility and/or skilled nursing facility required
- Minimum of 2 years med/surg experience in an acute care and/or skill nursing facility preferred
