Description
The Case Manager will work with individuals in Allegheny County who are diagnosed with an addiction to drugs and/or alcohol and are involved in the criminal justice system.
This position allows the opportunity for the ideal candidate to partner with the various departments within the agency, as well as external contacts, such as Justice Related Services and probation, all while helping to make a difference in the lives of others. The ideal candidate will posses strong organizational and customer service skills. A background in behavioral health substance abuse or case management preferred.
The starting rate for this role is $15.34, prior to crediting for any experience above the minimum requirements. The dress code is business casual and the primary location for this position will be in an office setting, with some off-site local involvement as needed. Responsibilities:
- Maintain productivity as stated in program goals and objectives.
- Monitor progress of clients and serve as a liaison between referral source and treatment/community linkages.
- Conduct screens on potential clients who are incarcerated in the Allegheny County Jail.
- Attend staff development and training as directed.
- Assess the treatment needs and the characteristics of each individual.
- Develop and maintain productive relations with community linkage.
- Complete all required documentation, type correspondence and compile required statistics.
- Refer individuals to appropriate treatment programs as well as other identified needed resources in the community.
- Identify drug/alcohol abusers in the criminal justice system with the potential for effective treatment outcomes.
- Assess case management and treatment needs of each individual.
- Collect supervised urine specimens from clients and/or breathalyzer results, document results and arrange for biohazard material disposal.
- Develop individualized service plans with each individual who is accepted as a client.
- Prepare monthly reports on client?s progress for submission to referral source. In cases where the client is in treatment, report will be prepared with consultation from the treatment team members.
- Transport clients as necessary, to appointment related and program goals.
Qualifications
- BA/BS in Social Services/Human Services or Criminal Justice.
- Minimum of 1 year experience in Drug & Alcohol, or Mental Health.
- Must be available to work one evening per week as needed.
- Possess ability to administer the PA Client Placement Criteria (PCPC) assessment.
- Must be able to obtain clearance to conduct TASC screens in the Allegheny County Jail.
