The HH Social Worker, MSW will plan, organize and implement social work services in accordance with Agency policies, procedures and Agency code of ethics. This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance.

  • Participate in Agency committees as requested.
  • Evaluate the effectiveness of the social work plan of care based on patient need(s) and response(s). Identify variances to establish goals/outcomes and work in collaboration with supervisory personnel to implement care/services to assure attainment or outcomes. Consult and coordinate with internal and external health care team members/sources. Refer patients to other allied community organizations or agencies for assistance with the needs, which cannot be met by the Agency services. Advocate for the patient/family in securing resources from health care providers. Document and communicate observations and evaluations through reports, records and conferences. Communicate and consult with patient's physician.
  • Possess Agency equipment required to perform evaluations and implementation of care as outlined in plan of care and procedures.
  • Adhere to Agency and standard principles of infection control.
  • Oversee services provided by Social Worker I.
  • Responsible for discharge of patients:Plan and prepare for discharge on an on-going basis, documenting all coordination activities and arrangements for on-going care. Coordinate referrals to internal/external resources to assure attainment of outcomes. Assure the discharge summary is accurate and complete. Complete discharge instructions for the patient at the time of discharge.
  • Participate in liaison, education and inter-agency planning.
  • Works to achieve and maintain quality and customer satisfaction in delivery of services to internal/external customers.
  • Provide social work services to patients and their families, or groups using techniques applicable to the social work role: Identify the social, emotional, and economic needs of patients through a continuous evaluation process. Establish a plan of care under the direction of the physician and in conjunction with internal Agency review processes. Implement the plan of care in accordance with objectives mutually established with the patient and family. Provide individual and group counseling services for patients to assist them in coping with their social, emotional, economic and health needs.
  • Self assess educational needs and initiate plan to maintain and attain general and specialty clinical competencies.
  • Participate in educational programs, orientation, quality assurance, and quality improvement activities.

  • Master's degree of Social Work from a school of Social Work accredited by the Council on Social Work education. 
  • Minimum of one (1) year of related work experience. 
  • Competency in computer skills in a Windows based environment. Demonstrate ability to perform computer skills in specialized computer application(s) as needed.

Licensure, Certifications, and Clearances:

  • Automotive Insurance
  • Driver's License 
  • Licensed Social Worker (LSW) 
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
