As a Registered Nurse Assessment Coordinator, you will assess the level of care needed for every resident on admission in collaboration with other members of the interdisciplinary team to ensure the physical, medical, and psychosocial needs of residents are met.
Responsibilities:
- Present notification to the billing office of changes in payor source, admission, transfers and discharges.
- Maintain current knowledge of regulatory requirements
- Coordinate and facilitate the Interdisciplinary Care conferences..
- Complete tracking forms for MDS ensuring timely completion and submission, reentry and discharge.
- Schedule and ensure notification of families/residents and team members when Care conference is scheduled.
- Assess Level of Care on every resident on admission and then on a continuing basis
- Ensure completion of the MDS and Assessment on admission, annually and with every change in Level of Care by relevant members of interdisciplinary team.
- Monitor specific data that affects CMI and ultimately reimbursement
- Contribute to completion and necessary updating of Plan of Care on admission, quarterly and with changes in Level of Care.
- Manage data collections for Case Mix groupings quarterly for MA residents and annually for residents.
- Perform requested reviews with third party payers, documenting interactions and clinical information provided to ensure no gap in covered days.
- Participate in Department of Health surveys, utilization management review process, communicating effectively with regulatory and field operation representatives.
- Tally and notify billing office of bed hold status for affected residents, and timely notification upon return
- Manage timely electronic submission of data to CMS weekly, monthly, quarterly and annually.
- Monitor data entered into MDS and Plan of Care for accuracy and completion.
- Collaborate with interdisciplinary team members in ensuring residents' needs are met and care is accurately and appropriately documented.
- Complete quarterly review on every resident
- Perform daily review of patients by payor class and communicate to the billing office.
- Ensure protection of Resident Rights.
- Follow established performance standards and performs duties prudently according to nursing policies and procedures
Qualifications
- 2 years nursing experience in LTC/Acute Care setting required
- Experience or training in assessing needs of LTC residents and development of care plans required
- 1 year RNAC experience preferred
- BSN preferred
Licensure, Certifications, and Clearances:
RAC-CT certification preferred CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
