The LPN will work rotating shifts, and occasional, but not required 12 hour shifts, depending on coverage needs for the facility.
Responsibilities:
- Provide emotional support and empathy to the guest through listening and encouragement.
- Provide physical and emotional support to the guest and family during the dying process.
- Attend compliance training and adhere to the organization standards of conduct, policies, and procedures
- Provide individualized end of life care to guests.
- Utilize standard blood and body fluid precautions.
- Demonstrates understanding of hospice philosophy of care.
- Maintain a safe, clean guest environment through observation and the performance of light household chores.
- Provide nutrition by preparing meals and assisting with food and fluid intake.
- Communicate appropriately with the Hospice IDT in a timely manner.
- Recognize guest emergencies and implement emergency procedures.
- Promote good personal hygiene by assisting the guest with bathing, shampooing, mouth care, nail care, and other personal care as appropriate.
- Maintain confidentiality regarding guest, staff and Hospice House matters.
- Maintain guest records and related reports
- Provide for maximum comfort to the guest through proper positioning and bed making.
Qualifications
- Minimum of two (2) years medical/surgical experience in an acute care facility and/or skilled nursing facility - Preferred
- Graduate of an accredited school for practical nursing-Required
- Effective communication and interpersonal skills- Required.
- Ability to work independently without constant, direct supervision- Required.
- Problem solving and critical thinking skills- Required.
