Description
This is a casual (as needed) position for the Adult Day Services department to help cover when staff is sick, scheduled off, or on vacation.
Responsibilities:
- Demonstrate support of Agency mission and philosophy in delivery of services.
- Assist with implementing a marketing plan that targets referral sources, and develops positive relationships.
- Perform specific direct care procedure after successfully obtaining competency.
- Responsible for documenting required information in member's record including attendance and participation.
- Assist with performance and quality improvement measures. Meet or exceed threshold regarding established quality indicators.
- Responsible for actively seeking out resources to assist with the development of the activities calendar in order to provide meaningful activities for the members.
- Responsible for administer medications to members as ordered by physician after successfully completing medication administration certification program.
- Conduct daily monitoring of the food program for compliance with Pennsylvania Department of Education guidelines.
- Participate in Agency committees as requested.
- Participate in required educational programs, staff meetings, training sessions, quality assurance program and quality improvement activities.
- Promote personal and co-worker safety during work duties.
- Assist with the coordination of care to facilitate admission into Adult Day setting. Establish relationship with member/family/caregiver. Work diligently to meet and exceed the initial needs/expectations of referred member/family.
- Take and record pulse, temperature, blood pressure and weight.
- Assist with customer relations and member satisfaction.
- Value members and their family's by making serving them a top priority.
- Communicate with referral sources in a courteous and timely manner.
- Initiate activities to resolve customer complaints.
- Report and document customer complaints.
- Establish and maintain customer relationship focus with all internal and external customers.
- Assist in the development, coordination and implementation of activities for Adult Day Services members in accordance with regulatory guidelines.
- Assist in the development and implementation of activities for socialization of members.
- Assist in the development of special needs activities for target groups and provide necessary assistance during activities.
- Assist in the development of monthly activities calendar and participate in monthly activities reporting.
- Arrange for materials required for specific activities.
- Assist with special needs of members during activities.
- Assist with the development of clinical conferences and progress notes. Insure care plan is developed to meet the needs of the individual members.
- Provide breakfast, lunch, and snacks prepared for the special nutritional needs of the members.
- Assist members in activities of daily living. Assist with walking and transferring. Assist with toileting, changing and feeding of members, as needed.
- Maintain and protect member and Agency confidentiality.
- Responsible for overseeing the coordination of transportation for members.
- Endeavor to achieve and maintain quality and customer satisfaction in the delivery of services to internal and external customers.
Qualifications
- High School Diploma or GED equivalent required.
- Minimum of one year related work experience preferred.
- Competency in computer skills in Window based environment.
- Demonstrates ability to perform computer skills in specialized computer application(s) as needed.
- CPR and First Aid certification required within one year of employment.
- Access to transportation in the Agency service area with valid Pennsylvania driver's license and auto liability insurance.
Licensure, Certifications, and Clearances:
HM/HHA or CNA certification preferredCPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
