Caregiver, Team Leader (VNA of Venango)

Description

Are you a caregiver looking to take your role to the next level, with a leadership position? If so, this is the perfect opportunity for you!
Provide hands-on practical training to field staff regarding personal care tasks, transferring, positioning, body mechanics, and safety while ensuring customer service standards are integrated into client care. Function ongoing as a mentor to staff to provide guidance when caring for clients with complex physical and emotional needs, ensuring that the team is confident and well trained to provide quality care to clients. Provide home care services to clients as designated in the plan of care by assisting with personal care, light housekeeping, shopping and errands, meal preparation and feeding, paying bills, escorting to appointments, and medication reminders. This will be done in a manner consistent with agency policies and procedures, customer satisfaction standards, and Department of Health and Office of Long Term Living regulatory requirements.

Responsibilities:

  • Perform all duties of a Caregiver including providing clients assistance with personal care, light housekeeping, shopping and errands, meal preparation and feeding, companionship, safety monitoring, medication reminders.
  • Performs other duties as requested by the home care supervisor.
  • Performs in accordance with system-wide competencies/behaviors.
  • Function as a mentor to staff during orientation and ongoing as needs arise regarding skill-based procedures or clients with complex needs.
  • Regularly attend staff meetings.
  • Provide hands on practical training to new employees during orientation to enhance their personal care skill performance and their approach to providing excellent customer service.
  • Report customer concerns/complaints to the home care supervisor for immediate follow up and resolution.
  • Complete ongoing and annual health screening and educational requirements to maintain competency according to regulatory requirements.
  • Conduct on-site observation visits in the field with staff within 30 days or hire and quarterly thereafter, as requested by the home care supervisor; document visits on the on-site observation visit form.
  • Responsible for ensuring accurate and timely documentation of care provided through effectively using the telephony system according to established procedures.
  • Responsible for observing and immediately communicating to the care supervisor any significant changes in a customer's condition, suspected cases of abuse or neglect, and safety concerns.

 

Qualifications

  • High School Diploma or GED required.
  • 1 year of related work experience and CNA required or 2 years of related work experience required.
  • Access to transportation in the Agency service area with valid Pennsylvania driver's license and auto liability insurance.

  • Automotive Insurance
  • Driver's License
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance
  • OAPSA
