UPMC Tioga County Partnership has an exciting opportunity available and is seeking a dynamic full time Partnership Program Coordinator to coordinate activities of the Partnership program including health plan activities, ensures representation from all Partnership members and oversees the implementation of collaborative initiatives designed to meet the health and human service needs throughout Tioga County.

 

Successful candidate will possess excellent written and oral communication skills; experience working with children; classroom instruction experience; basic computer skills using Word and Excel; experience with planning events.

 

Occasional evening and weekend hours (up to 12 times per year) are necessary to accommodate community events or prevention programs.

Responsibilities:

  • Incorporate leading/evidence-based practice in quality/safety into every day work. Commit to high quality, safe work; encourage others to have similar standards. Identify processes/systems that could lead to inefficient practices, unsafe conditions, errors or adverse events. Avoid shortcuts to ensure quality and safety measures are not circumvented. Encourage patient and family involvement in processes involving patient safety. Speak up with other service partners to ensure high quality and safety. Report serious events/incidents per policy.
  • Mentors interns assigned to the Partnership through introduction to the work group members and providing constructive feedback on efforts to participate in various activities.
  • Convenes and facilitates work group meetings and activities. Ensures that appropriate resources are available for groups to be successful in their endeavors to improve community health.
  • Know and follow all organizational/departmental policies/procedures.
  • Assume responsibility for personal development and education.
  • Participates in meetings of the Partnership including planning, Finance, Executive and full Board meetings to report out on the activities of the work groups.
  • Performs identified tasks to implement Communities That Care project including communicating with the state level consultant for the grant.
  • Represents the Partnership in community events, such as health fairs, educational programs, Town Hall meetings and any other activities offered by other non-profits.
  • Submits required reports to supervisor. Communicates ideas for implementation and improvement to supervisor.
  • Participates in strategic planning activities and demonstrates efforts to achieve agreed upon measures to achieve goals.
  • Actively and purposefully pursues attainment of project goals to meet deliverables as outlined in grant-funded programs.
  • Actively practice behaviors associated with SH Service Excellence expectations. Smile at and greet everyone enthusiastically, treat everyone with respect, dignity and C.A.R.E. Be an ambassador for Susquehanna Health. Provide the best C.A.R.E. for our patients no matter what.

 

Qualifications

Bachelors degree in Public Health Administration, Community Health, Health Education, Education, Social Work, or other health and human services field required. Two years experience working with the public required. Knowledge of community issues, health risk factors and local health care and social service programs required.

Licensure, Certifications, and Clearances:

  • Act 31 Child Abuse Reporting with renewal
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal

    Salary Range: $19.66 to $29.49 / hour
