Description

Purpose:
The High Fidelity Wraparound (HFD) Youth Support Partner will work as part of the High Fidelity Wraparound team to assist youth with Type 1 Diabetes (T1D), and their families. The Youth Support Partner will provide various levels of information and direct support to youth, ages 12-15 with T1D, to help strengthen their knowledge of living with T1D and to develop their natural support system. The Youth Support Partner will work closely with the HFW Facilitator and the HFW Youth Support Partner to support positive outcomes for the youth and family.

Responsibilities:
  • Assure that youth voice and choice are heard and respected
  • Provide direct information and support to youth with T1D, as needed, understanding that only the medical care team at Children?s Hospital provides recommendations in medical management.
  • Implement the 10 Principles and the 4 Phases of HFW and ensure that HFW is delivered with fidelity to the model.
  • Attend all mandatory trainings and become a credentialed HFW Youth Support Partner within one year.
  • Assist the youth in linking to natural, school, and community supports
  • Work with and help youth and families to learn skills, develop confidence, to achieve their vision, and to become self sufficient
  • Work closely with other HFW team members to assure a coordinated approach
  • Seek coaching, supervision and professional input regularly and when needed
  • Assist with crisis stabilization when needed
  • Work with youth and families to create and maintain a team approach
  • Other non-essential duties as specified by the immediate supervisor

Qualifications

  • Must have personal experience in successfully managing one's own T1D with a willingness to share personal information and experiences as appropriate.
  • Must be between the ages of 18 and 25
  • Must have a high school diploma or GED
  • Possess life experiences that have allowed the formation of healthy working relationships with youth, families, clinicians, team members, and/or others.
  • Possess some relevant knowledge of existing community and natural supports which provide youth with T1D the opportunities to succeed
  • Knowledge of the High Fidelity Wraparound process helpful but not necessary
  • Ability to engage youth (ages 12-15) with T1D, to identify strengths without being judgmental, and to integrate these strengths throughout the provision of services
  • Ability to role model effective behaviors and appropriate skills
  • Ability to support and improve self-efficacy and confidence
  • Ability to promote and strengthen healthy relationships
  • Ability to assist others working with the adolescent to understand adolescent culture 
  • Possess strong verbal and written communication skills
  • Willing and eager to learn, and to be videotaped for credentialing and training purposes
  • Excellent documentation skills
  • Excellent time management and organizational skills
  • Flexible in adapting the weekly schedule to meet the needs of the youth, families and the team

Licensure, Certifications, and Clearances:
  • Driver's License
  • Act 31 Child Abuse Reporting
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance
