Description
Purpose:Responsible for performing select activities related to the clinical management of infusion and enteral therapy patients including patient supply management, coordinating services with nursing and operations and processing refills under the supervision of a licensed pharmacist.
Responsibilities:
- Performs 48 hr - 5 day follow-up telephone calls to all new patients.
- Utilizes appropriate communication lines in relaying problems, concerns, questions and ideas.
- Prioritizes and completes work on schedule.
- Carries out all responsibilities in accordance with Chartwell's Core Values.
- Utilizes the patient Service Delivery log as per SOP.
- Shows pride in work.
- Trends laboratory results and forwards to Pharmacist.
- Handles client complaints in a fair and empathetic manner.
- Makes sure that work meets quality standards.
- Coordinates/Schedules deliveries of patient orders with pharmacy operations.
- Communicates verbally with patients, caregivers, nursing staff, Patient Services Representatives or other agencies to monitor patient supply inventories and determine specific patient supply needs.
- Cooperates with other staff members when planning and organizing daily activities.
- Takes initiative to present ideas/suggestions to management.
- Good organizational skills.
- Processes packing slips for refill patient orders.
- Demonstrates reliability and follow-through on all assigned tasks.
- Gives high priority to client satisfaction and customer needs.
- Maintains confidentiality of patient information.
- Demonstrates ability to communicate effectively and express ideas clearly.
- Sets high goals or standards of performance for self.
- Demonstrates the ability to work effectively and maintain expected productivity.
- Processes RX refills under the supervision of a Pharmacist.
- Evaluates product usage and updates/revises patient inventory usage numbers in the computer based on actual usage after consultation with nursing, outside organizations and the clinical pharmacist.
- Other assignments as requested by supervisor.
- Maintains courteous demeanor at all times.
Qualifications
High School Diploma required. Pharmacy Technician training preferred. Previous customer service experience helpful. Basic knowledge and experience in homecare desirable, good organizational and time-management skills. Excellent communication skills.Licensure, Certifications, and Clearances:
Pharmacy technician certification preferred.
