The Jameson Child Advocacy Center is currently hiring a Full Time, grant-funded Nurse Home Visitor to support the Nurse-Family Partnership program.

The Children's Advocacy Center of Lawrence County houses Early Headstart, Parents as Teachers, Nurse-Family Partnership, Fatherhood, Trauma Focused Therapy, and Forensic Interviewing and Examination programs. Our focus is child abuse and prevention and education.

The new team member will work in the Nurse-Family Partnership program. This nationwide program has three goals for our clients:
• Healthy pregnancy
• Healthy baby
• Self-efficacy
Our clients are first time, low income mothers and their babies.

This position will require you to attend a week long training in Colorado. Shifts for this position will be primarily daylight shifts, with 100% travel (reimbursed). This position is home-based, with 25-27 cases. This position will not require on-call.

The Nurse-Family Partnership nurse home visitor is responsible for providing comprehensive nursing services to women and their families eligible for the Nurse-Family Partnership Program. The nurse home visitor is responsible for maintaining the highest standards in clinical nursing practice and adherence to the Nurse-Family Partnership model, policies, procedures, Visit-to-Visit Guidelines, and standards of Nurse-Family Partnership and of the lead agency. The nurse home visitor reports to the Nurse-Family Partnership nurse supervisor.

  • Provides education, support, and referral resources in assisting women and their families in attaining their targeted goals. Consults and collaborates with other professionals involved in providing services to women and families. Formulates nursing diagnosis based on nursing assessment and client goals
  • Provides home visits to women and their families eligible for the Nurse-Family Partnership Program. Adheres to nursing process and the Nurse-Family Partnership model of home visitation. Supports Nurse-Family Partnership policies, procedures, Visit-to-Visit Guidelines, and standards of Nurse-Family Partnership and the CAC.
  • Performs home visiting in accordance with the Nurse-Family Partnership model and Visit-to-Visit Guidelines. Assesses physical, emotional, social, and environmental strengths and risks of women and their families as they relate to the Nurse-Family Partnership domains. Assists women and their families in establishing goals and outcomes.
  • Evaluates client progress toward stated goals and Nurse-Family Partnership outcomes. Plans home visits in accordance with client goals and Nurse-Family Partnership outcomes Provide information to support staff for timely and accurate data input to the Nurse-Family Partnership data system. Participate in quality improvement efforts.
  • Participates in community coalition in support of Nurse-Family Partnership. Promotes public awareness of Nurse-Family Partnership. Develops and maintains community relationships to support client referrals. Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.


RN with six months relevant experience required.  
BSN required **BSN requirement may be temporarily waived in the event the grant program provides a variance. The employee would be required to obtain the degree within 36 months of hire if the variance is granted.  
Two years recent experience in maternal/child health, public health, home visiting, or mental/behavioral nursing preferred. 
Home visiting experience preferred. 
Excellent written and verbal communication skills. 
Ability to utilize Microsoft Office.
Must complete Nurse Family Partnership initial training in Denver, Colorado.

Licensure, Certifications, and Clearances:

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Child CPR required
  • Registered Nurse (RN)
  • Driver's License
  • Valid Car Insurance
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance
