Responsibilities:
The part-time, 16 hour, Community Social Service Coordinator will work out of Homestead High Rise and assist with seniors. This position serves mainly as a social worker who tries to answer any questions they may have, or refer them to services for seniors.
This position will work 2 days a week, and those can be flexible. The hours are 8:00 a.m. - 4:30 p.m., but those are flexible as well.
Responsibilities:
- Act as a resource person as requested by clients/agencies
- Act as patient advocate.
- Analyze the types, frequency and other characteristics of services that residents use, need and want.
- Assist family members to understand the changing needs of aging in place.
- Assist seniors with limited financial resources to age in place
- Attend age/client population specific in-services
- Case manage senior citizens living in senior citizen High Rises
- Communicate appropriate information to other members of the outreach team
- Communicate concise and appropriate information concerning patients needs and condition verbally and in writing.
- Conduct and find others to conduct workshops or educational activities for the residents based on relevant topics and emerging changes in the health care field that effect seniors.
- Cooperate with other hospital departments.
- Develop individual service plans for residents upon request.
- Develop programming/activities designed to increase participation by residents.
- Educate service providers on needs of resident population and ensure affordability.
- Establish evidence-based program monitoring metrics
- Help arrange financial management assistance
- Help families utilize services in community
- Help residents obtain equipment and devices
- Identify and coordinate services in the community and their availability to residents.
- Keep constantly updated on policy changes that impact the lives of consumers
- Liaison between residents and social service agencies.
- Maintain established office hours at teach senior high rise
- Maintain monthly statistics necessary for monitoring success of program (visits, type of services, documentation of education presentations and other statistics as requested).
- Make self-available to listen to patient and family concerns.
- Network with other service providers to provide the optimal care to seniors with limited incomes.
- Order necessary office supplies.
- Promote and maintain positive working relationships with outreach team.
- Represent UPMC at community events and service programs as assigned
- Respond to crisis situations by coordinating appropriate services to enable senior to remain in their current living situation (if possible).
- Seek opportunities to improve process, streamline work and track outcomes.
Qualifications
- Bachelor's degree in Social Work or Psychology required
- Minimum three (3) years of experience working with geriatric clients in a community setting
- Equivalent combination of education/experience will be considered
- Extensive knowledge of community based services including services offered by the Department of Aging and the Department of Welfare
- Ability to work with seniors and limited incomes
- Guardianship experience
- Knowledge of the various types of senior housing
- Knowledge of public housing
- Knowledge of the various levels of care
- Knowledge of the OPTIANS process
- Knowledge of the COMPASS system
- Able to coordinate programs between agencies
- Community building skills, motivated, able to work independently, self-starter, negotiations skills, conflict resolution skills, and the ability to get along well with seniors
Licensure, Certifications, and Clearances:
- Act 34 Criminal Clearance
- Older Adult Protective Services Act
