This is a Full Time position on the Daylight Shift.
Responsibilities:
- Provides resident/significant other health teaching as delegated by and observed by the RN.
- Administers medication and treatments accurately, observing resident response, as evidenced by documentation in the medical record and lack of negative outcomes.
- Monitors I.V. sites and administers I.V. meds through peripheral lines if LPN has basic I.V. certification.
- Documents all aspects of residents care as accurately, concisely, and promptly as per state and federal requirements
- Maintains and coordinates communication among resident/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback
- Attends physician rounds on assigned residents and communicates observations - regarding residents condition
- Maintains confidentiality of resident information at all times.
- Demonstrates knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals.
- Complies with the facilitys infection control policies and procedures including Body Substance Isolation (BSI) and the Exposure Control Plan that reflects OSHA Bloodborne Pathogens Standard to ensure a safe working environment for self and others.
Qualifications
Current Licensure as Licensed Practical Nursing in Pennsylvania or temporary practice permit required.
Minimum one-year experience in long term care or other clinical nursing experience preferred.
