Mobile Therapist/MH Profess

Description

Purpose:
The Mobile Therapist/MH Professional provides mobile assessment, treatment planning, and implementation of a behavior modification intervention plan, which is individualized to each consumer. Provide direct clinical service and support to consumers in the community. Services are provided as prescribed by psychiatrist and in accordance with applicable regulations.

Responsibilities:

  • Model Behavioral Plan Implementation to facilitate acquisition of skills by treatment team members
  • Provides assessment, support, resources, and behavioral modification to consumers in the community.
  • Complete thorough and meaningful treatment plans and written documentation of all contacts according to established documentation requirements.
  • Complete all required documentation and billing in a timely fashion and consistent with Behavioral Health procedures
  • Provides direct clinical services via individual therapy and demonstrates appropriate clinical counseling skills which include family members in the formulation, development, implementation and monitoring of treatment efforts.
  • Demonstrate internal and external quality customer service standards to all that request assistance, responding in a professional manner.
  • Participate in inter-agency planning team meetings and other organized, relevant exchanges of clinical information as requested and authorized by the Program Director or her designee
  • Adhere to all legal and ethical standards, and principles of confidentiality as prescribed by professional clinical standards and the Policies and Procedures of Western Psychiatric and Institute and Clinic
  • Adhere to all Mobile Mental Health Therapy (MMHT) regulations.
  • Develop evidence based behavior plans which adhere to the principles of strengths based, least restrictive, least intrusive clinically sound interventions
  • Identifies behavioral goals and intervention techniques, and recommends non-aversive behavioral change methods as well as treatment plans with collaborative goals constructed with the consumers. The mental health professionals on the Mobile Mental Health Therapy team will work to provide direct in-home care to consumers after a crisis assessment, emergency evaluation, or post-discharge from an inpatient hospitalization.
  • Complete thorough and meaningful treatment plans and written documentation of all contacts according to established documentation requirements.
  • Demonstrate sound clinical insight and knowledge.
  • Assesses the frequency, intensity, and/or duration of target behaviors. Defines target behaviors in measurable ways.
  • Determine appropriate data tracking systems to monitor progress and assess efficacy of specific treatment strategies
  • Provides direct clinical services via individual therapy and demonstrates appropriate clinical counseling skills which include family members in the formulation, development, implementation and monitoring of treatment efforts.
  • Demonstrate sound clinical insight and knowledge.
  • Attend Regulated Supervision requirement; face-to-face supervision with supervisor at least once per month for one hour.Demonstrate internal and external quality customer service standards to all that request assistance, responding in a professional manner.
  • Attend Regulated Supervision requirement; face-to-face supervision with supervisor at least once per month for one hour.

 

Qualifications

  • Master's degree in social work, psychology, counseling, education, nursing or related human service field and at least one (1) year of mental health direct care experience required.
  • Training in behavioral modification, motivational interviewing, solution-focused therapy in behavioral modification techniques preferred.
