The Senior Registered Nurse Assessment Coordinator is responsible for coordinating assessments of residents by the interdisciplinary team and the need for revisions to their Plans of Care; completing and submitting the Minimum Data Set ensuring accuracy and timeliness to promote optimal reimbursement for care provided in accordance with current standards, guidelines, regulations, and internal policies and procedures. The additional responsibilities of the Sr. RNAC include serving as a mentor to other RNACs, serving as the subject matter expert and resource person, representing RNACs on various committees, and providing requested information to the corporate office.

  • Assist CNO with coordination of RNAC team meetings.
  • Participate in recruitment and selection of RNACs as needed.
  • Participate in continuing education programs and professional networking activities.
  • Assess Level of Care on every resident on admission and then on a continuing basis Perform requested reviews with third party payers, documenting interactions and clinical information provided to ensure no gap in covered days. 
  • Perform daily review of patients by payor class and communicate to the billing office
  • Schedule and ensure notification of families/residents and team members when Care conference is scheduled.
  • Manage data collections for Case Mix groupings quarterly for MA residents and annually for residents. 
  •  Manage timely electronic submission of data to CMS weekly, monthly, quarterly and annually. 
  •  Monitor specific data that affects CMI and ultimately reimbursement
  • Ensure completion of the MDS and Assessment on admission, annually and with every change in Level of Care by relevant members of interdisciplinary team. 1
  • Contribute to completion and necessary updating of Plan of Care on admission, quarterly and with changes in Level of Care.
  • Monitor data entered into MDS and Plan of Care for accuracy and completion. 
  •  Complete tracking forms for MDS ensuring timely completion and submission,; reentry and discharge. 
  •  Assist in the orientation, training, development and evaluation of nursing employees on MDS, care plans and documentation requirements.
  • Maintain current knowledge of regulatory requirements follow established performance standards and performs duties prudently according to nursing policies and procedures Ensure protection of Resident Rights. 
  • Collaborate with interdisciplinary team members in ensuring residents needs are met and care is accurately and appropriately documented.
  • Complete quarterly review on every resident
  • Participate in Department of Health surveys, utilization management review process, communicating effectively with regulatory and field operation representatives. 
  •  Present notification to the billing office of changes in payor source, admission, transfers and discharges. 
  •  Tally and notify billing office of bed hold status for affected residents, and timely notification upon return
  • Coordinate orientation for new/supplemental staff members to the RNAC role. 
  • Attend meetings as the RNAC representative including but not limited to: monthly case mix management.
  • Facilitate gathering information for CNO reports including Quality Measures, CASPER, Reports, PEPPER Reports, etc.Provide educational information for clinical staff related to medical record documentation and ADLs to support the MDS assessment. 
  • Provide education for staff members related to MDS assessment, coding Serve as the super user, trouble-shooting and assisting with electronic health record implementation, testing, and making any revisions in the MDS portions.
  • Serve as the subject matter expert and resource to other RNACs, providing updated information re: MDS, RAI manual, and other regulatory information.


  • 3 years nursing experience in LTC/Acute Care setting required.
  • Minimum 1 year of experience as an RNAC required 
  • BSN preferred 
  • UPMC approved certification required   
  • Nurses holding a Master's degree are exempt from certification
  • Professional contribution required

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hireRAC-CT certification required

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)
  • Act 34
