Description
Assist residents with those activities of daily living that they are unable to perform without help, fostering at all times residents' independence and freedom of choice
Responsibilities:
- Actively practice behaviors associated with SH Service Excellence expectations. Smile at and greet everyone enthusiastically, treat everyone with respect, dignity and C.A.R.E. Be an ambassador for UPMC Susquehanna. Provide the best C.A.R.E. for our patients no matter what.
- Monitors appropriate use of resident car
- Conducts room checks and resident rounds monitoring for environmental hazards recording all incidents, errors or accidents.
- Assume responsibility for personal development and education
- Know and follow all organizational/departmental policies/procedures
- Provides assistance to residents with daily activities including but not limited to: dressing, bathing and personal hygiene, cleaning, laundry, meal preparation.
- Commit to high quality, safe work; encourage others to have similar standards.
- Identify processes/systems that could lead to inefficient practices, unsafe conditions, errors or adverse events.
- Avoid shortcuts to ensure quality and safety measures are not circumvented.
- Encourage patient and family involvement in processes involving patient safety.
- Understand that patient safety is everyone's responsibility.
- Assume responsibility for personal education and performance related to patient safety; report unsafe conditions or events immediately; speak up in a non confrontational and educational manner when seeing a safety breech; develop or participate in a project or action plan to reduce risk or mitigate harm for patients, as appropriate
- Delivers mail and assists residents with personal correspondence
- Observes and reports changes in residents' physical condition and cognitive/emotional status to Personal Care Home Administrator
- Escorts residents and coordinates internal transportation needs providing assistance with mobility devices including the monitoring of safe transfers and ambulating techniques
- Prepares and maintains resident census records
- Assists residents with self-administration of medications including monitoring medication supply and documentation.
- Participates in the resident-centered activity program
Qualifications
High School Diploma or equivalent Required.Licensure, Certifications, and Clearances:
