Responsibilities:
- Responsible for maintaining schedule/calendar of patient visits to ensure all patients are scheduled as necessary Monitor float log and assure all patients are scheduled
- Monitor float log and assure all patients are scheduled
- Maintain Therapy Referral Log and compare with Episode Discipline Referral to ensure timeliness of service provision
- Coordinate scheduling process utilizing Intake Log for new referrals to ensure all patients appear on the schedule
- Inform clinical supervisor, as necessary, when staff availability is limited.
- Maintain and protect customer and Agency confidentiality
- Educate and require adherence to performance improvement processes and initiatives
- Maintain personal productivity by demonstrating satisfactory attendance and punctuality
- Demonstrate support of the Agency's Mission, Principles, and Values, Ethics, & Standards of Behavior
- Expand knowledge and expertise through educational resources and literature review
- Responsible for scheduling new referrals for patient assessments/evaluations under the direction of the clinical supervisor.
- Confirm physician orders exist for therapy evaluations
- Schedule clinician block time, as directed by supervisor, for meetings and/or scheduled education to ensure schedule accuracy
- Assure correct service type is scheduled for visits
- Ensure all designations are assigned to the appropriate clinician (primary, secondary, case manager)
- Report all variances to supervisor and follow up as directed
- Represent and participate in Agency professional and community activities as requested
- Model and mentor effective leadership and management
- Ensure clinicians and/or aides are aware of schedule for the following day including changes (additions or deletions), new referrals/assignments and specific notations unique to the patient
- Follow end-of-day processes for communication with staff. Notify supervisor of any variance in communication expectations.
- Promote personal and co-worker safety during work duties
Qualifications
- Education: Post High school education required. Associate degree or LPN preferred. In lieu of educational requirements, will consider 3 years of scheduling experience.
- Experience: Minimum two years related healthcare experience
- Computerization Skills: Proven competency in EXCEL, Microsoft Office.
- Demonstrated ability to perform computer skills in specialized computer application(s) as needed.
Licensure, Certifications, and Clearances:
Certification/Licensure: State licensing, as applicable, if an LPN
