Description

UPMC Pinnacle is currently hiring for a Patient Safety Specialist at the UPMC Memorial Hospital in York, PA. The hours for this position are primarily day shift but may include some evenings as needed. The individual hired for this role may be assigned to other UPMC Pinnacle hospital sites as they gain experience in the role. This position will be responsible for the development, implementation and monitoring of the UPMC Pinnacle patient safety program at the respective UPMC Pinnacle facility.
 
Responsibilities:
·         Implements the organization’s patient safety plan to achieve state patient safety objectives and goals.  
·         Investigates actual and potential patient safety events that occur at the assigned organization(s) through medical record reviews, interviews, physical inspection, and other means as necessary.
·         Ensures submission of event reports in accordance with state and federal law and regulation.
·         Ensures adherence to all patient safety state and federal patient safety laws and regulations.
·         Reviews actual and near miss patient safety events with physicians, senior leaders, managers, and frontline line staff to identify system vulnerabilities and opportunities for process improvement.
·         Conducts gap analyses and proactive failure mode effects analyses as the situation dictates.
·         Assists in ensuring compliance with National Patient Safety Goals established by The Joint Commission, including the review and revision of policies and procedures as needed.
·         Facilitates intensive reviews, debriefings, and root cause analyses for patient safety events, including never events, sentinel events, and near misses.
·         Assists in ensuring that any actions identified as a result of those reviews are assigned and implemented.
·         Ensures that significant patient safety events are communicated to the designated patient safety officer and other leaders through established mechanisms.
·         Identifies concerning trends and process vulnerabilities that need to be addressed to prevent patient harm and works with the designated patient safety officer and other leaders to address those concerns.
·         Presents patient safety education to various audiences on an ongoing basis.
·         Serves as an expert educator and consultant on Just Culture, including providing the Just Culture education and training and facilitating Just Culture champion and open forum meetings.
·         Serves as a role model to foster a culture of patient safety across the organization.
·         Ability to interact with patients and families in the disclosure and discussion of patient safety events.
·         Interfaces with risk management functions and legal counsel as needed.
·         Staffs the organization’s patient safety committee and participates on other hospital committees where patient safety expertise is required.
·         Participates in the hospital-wide performance improvement efforts through data gathering and analysis.
·         Works cooperatively with other members of the department to meet common goals.

Qualifications

  • Bachelor's degree in Nursing, Business Administration, Finance, Accounting or related healthcare area required.
  • 5 years of Clinical experience OR experience in risk, liability or loss control at the organizational level is required.
  • Ability to work with a variety of Microsoft office software systems.
  • Analytical ability to prepare, review and present statistical data.Interpersonal skills to work with management and physicians, and all levels of staff.

Licensure, Certifications, and Clearances:
Certification in Quality Management from Health Care Quality Council Management preferred

  • Registered Nurse (RN)
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
