Purpose:
Supervises front line staff in Home Health, assuming responsibility for their work performance and providing oversight of work flow and processes. Responsible for all clinical activities of assigned nursing and other professional staff. Oversees all case management activities and meets or exceeds established outcome measures.

Responsibilities:
  • Establish evidence-based protocols to guide effective practice.
  • Ensure all productivity and documentation standards are met.
  • Coordinate education for staff, patients and families.
  • Supervises frontline staff in Home Health, assuming responsibility for their work performance and providing oversight of work flow and processes. Responsible for all clinical activities of assigned nursing and other professional staff.
  • Work directly with staff on patient experience, quality and cost measures to meet or exceed established targets.
  • Rewards, disciplines and evaluates direct reports, using independent judgment; provides input into hiring and involuntary termination decisions.
  • Complete field visits at least monthly to assess care, service and patient satisfaction.
  • Evaluate patient care and outcomes through effective utilization of reports.
  • Review supply chain management protocols daily.
  • Provide input into department?s budget; supports the financial goals of Susquehanna Health through individual actions and management of resources.

Qualifications

  • Two years of home health experience or two years of supervisory experience in a healthcare setting required.

Licensure, Certifications, and Clearances:
Physical Therapist licensure in the Commonwealth of Pennsylvania

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Physical Therapist (PT)
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance
