Description

This is a full-time Pediatric Home Connect Manager position for our Pediatric Homecare department.  Seeking an experienced Registered Nurse (RN) with Homecare and/or Pediatric experience.  Oncall coverage rotation required.

Purpose:

Responsible for overall planning and management of the Pediatric Home Connection program. Organize and directs program development, ongoing functions, and performance improvement initiatives. Must have excellent communication and organizational skills.

Responsibilities:
  • Develop the program's orientation and ongoing education activities to achieve and maintain staff clinical competencies and the implementation of clinical best practices
  • Ensure the employment of qualified personnel, adequate staff education and performance evaluations (G134)Implement a competency evaluation process to manage, track and trend competency data for nursing and paraprofessional staff to assure quality of service and compliance with regulatory organizations.
  • Facilitate the implementation of educational opportunities related to quality and customer satisfaction program initiatives.
  • Manage the orientation process of new hires and ensure staff are competent to meet patient needs.Improve the strength of the program through effective hiring, staff development and mentoring opportunities.
  • Achieve and maintain status as an expert leader within the program and the industry
  • Obtain and maintain mastery of licensing and accreditation standards
  • Maintain knowledge of advances and/or changes in industry and clinical best practices to ensure the attainment of clinical and operational initiatives
  • Responsible for Compliance and Ethics as it relates to services
  • Comply with all applicable laws, regulations and Agency policies/procedures
  • Complete education requirements
  • Educate and require adherence to performance improvement processes and initiatives
  • Model and mentor effective leadership and management
  • Develop and direct initiatives to improve Customer Satisfaction and ensure the program is the provider of choice in all regions

  • Assist in the development of annual budget and manage the program to achieve positive results through
  • Minimizing overtime and nurse travel Maximizing utilization of authorized hours of care

  • Manage the ordering and appropriate use of medical and office supplies.

  • Represent and participate in Agency professional and community activities as requested
  • Assist in the design, implementation and evaluation of the service delivery model to ensure program integrity, effectiveness and compliance with industry and licensing standards. Ensure that the service delivery model:Incorporates clinical best practices
  • Establishes a framework to achieve positive clinical, operational, financial and customer satisfaction outcomes
  • Can be implemented in any geographic region
  • Maximizes the use of technology
  • Assures a person-centered approach to care
  • Oversee clinical risk management processes and ensure appropriate response and follow up Manage all patient safety events and significant customer complaints
  • Report appropriate patient safety events to state agencies, as required.Conduct root-cause analysis as necessary and manage clinical remediation or education as a follow up
  • Maintain and protect customer and Agency confidentiality
  • Direct and oversee the provision of quality clinical services.
  • Determine admission to the program based upon established Agency policies/procedures
  • Oversee hiring to ensure the program's ability to meet patient needs
  • Assure skilled nursing and other therapeutic services furnished are under the supervision and direction of a physician or registered nurse.Oversee the development and implementation of administrative and program policies, procedures and standards of care.
  • Assure appropriate staff supervision during all operating hours
  • Assume a leadership position in managing the human resources of the program
  • Recruit, hire and retain qualified personnel to achieve program initiatives
  • Mentor leaders within the program to ensure Agency standards of care are fostered and achieved throughout all levels
  • Foster an environment to facilitate change
  • Establish and maintain effective channels of communication
  • Analyze staffing utilization patterns and make appropriate recommendations Identify and implement strategies to enhance employee satisfaction.
  • Develop, implement and evaluate a process to manage the authorization of services for various program payers. 
  • Maintain current knowledge of funding sources and managed care standards and practices for admission, service, and discharge 
  • Effectively manage and appeal inquiries or denials from third party payers
  • Establish and maintain effective communication with payer liaisons to ensure alignment of Agency procedures with payer expectations and to serve as an advocate for the patient's service needs
  • Develop, direct and monitor organizational Performance Improvement (PI) activities.Identify PI opportunities and implement strategies to continually improve performance
  • Report all safety problems and injuries appropriately
  • Promote personal and co-worker safety during work duties
  • Responsible to ensure ongoing compliance with HCSIS and PROMISe enrollment and SSD maintenance and accuracy.
  • Expand knowledge and expertise through educational resources and literature review
  • Direct initiatives to promote program growth
  • Develop and foster relationships with physician practices and tertiary care centers
  • Complete a market analysis in conjunction with key agency leaders that is in line with strategic initiatives and priorities
  • Demonstrate support of the Agency's Mission, Principles, and Values, Ethics, & Standards of Behavior
  • Maintain personal productivity by demonstrating satisfactory attendance and punctuality
  • Responsible for ensuring compliance with regulatory and/or accrediting organizations.
  • Manage and oversee the completion of quarterly clinical chart audits in order to assure compliance with regulations and established clinical practices. 
  • Prepare reports identifying areas of performance improvement.
  • Oversee the process of responding to payer inquiries, denials and/or audits.
  • Establish program policies and/or procedures to ensure that consistency and standardization is achieved.

Qualifications

  • Education: 
  • Registered Nurse, Bachelor's degree required. Master's degree preferred.
  • Experience: 
  • 3 years of Clinical experience required, pediatric experience preferred. 
  • Minimum 2 years experience in a management/leadership position.
  • Computerization Skills: Competency in computer skills in a Windows-based environment
  • Travel: Access to transportation in the Agency service area with valid drivers license and auto liability insurance.

Licensure, Certifications, and Clearances:
Certification/Licensure: Licensed as a professional nurse CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
