UPMC Home Health is currently hiring a full-time Clinical Coordinator.


This role does require a weekend and holiday shift rotation.

Purpose:
Accountable for proactively supervising, guiding, and monitoring activities of the multidisciplinary team to affect quality patient care through accurate documentation, ongoing education and coordination of the team?s operation. Provides direction and oversight to ensure safe levels of staffing while maintaining efficient patient throughput. Serves as a resource for all staff and is able to apply competent knowledge and skills to achieve quality outcomes.

Responsibilities:
  • Identifies opportunities for clinical quality as well as workplace improvement. Applies evidence based practice as a regular aspect of professional practice
  • Builds and mends relationships among colleagues within the framework of a healthy workplace. 
  • Role models keen cultural awareness including preventing and addressing horizontal violence and impairment.

  • Analytical ability necessary to address and improve clinical issues and assist with complex problem solving.

  • Superior interpersonal and communication skills sufficient to effectively create and maintain a positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families in both written and verbal communication.
  • Demonstrates good judgment and analytical ability to oversee and coordinate patient care in collaboration with the physician and the multidisciplinary health care team. 
  • Uses critical thinking skills to effectively advise and direct health care providers in their delivery of patient care while maximizing the efficient use of human and financial resources to achieve productivity standards.
  • Responsive to all departments daily; accountable to establish professional and respectful interpersonal relationships.
  • Facilitates teamwork among all departments involved in the patient care process. 
  • Seeks opportunities to share expertise with other members of the healthcare team within and beyond the clinical unit/department. 
  • Regularly seeks feedback on performance and applies continuous professional improvement. 
  • Supports continuous learning in the clinical area by sharing research and materials from professional organizations, presenting on the unit, supporting journal clubs, etc
  • Independently suggests and supports changes within the department/organization Involves staff and key stakeholders to achieve optimal patient experience and efficient patient throughput. 
  • Uses appropriate interpersonal styles and techniques to gain acceptance of ideas or plans; modifying one's own behavior to accommodate tasks, situations and individuals involved. 
  • Supports staff during times of difficult transitions.  
  • Participates in supporting mechanisms and systems that ensure the provision of effective patient care while utilizing resources appropriately. 
  • Adhere to reporting elements and requirements set forth by all regulatory agencies for patient safety.

Qualifications

  • BSN preferred 
  • Minimum of 2 years healthcare experience- required
  • Minimum of 1-year home health (or hospice) experience- preferred
  • Minimum of 1 years progressive leadership experience with relevant clinical experience preferred

Licensure, Certifications, and Clearances:
Current licensure as a professional nurse in PA_-Required
Current Certification in basic CPR- Required
Current drivers' license in state of residence- Required
Current auto insurance in state of residence- Required
Act 33/34 Clearances- Required
Act 73 required for Pediatric/OB caregivers- Required

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Registered Nurse (RN)
