UPMC St. Margaret Pharmacy Residency Program is hiring a casual Pharmacy Technician. This position will support the Transitions of Care program. Apply today to learn more!

 

Purpose:
The Pharmacy Technician, Associate assists the team by assisting with outreach and medication reconciliation. The Pharmacy Technician, Associate services are provided in compliance with Department of Health, The Joint Commission, and State Board of Pharmacy regulations.

Responsibilities:

  • Track all hospital and nursing home discharges and maintain an accurate spreadsheet of all eligible patients.
  • Complete scripted transitions of care calls within 2 business days of discharge under the guidance of our staff pharmacist.
  • Make follow up patient appointments within 7-14 days of discharge.
  • Track metrics under the guidance of the transitions team
  • Assist pharmacists and nurses with refills, demonstrating a high level of accuracy and skill.
  • Develop and maintain a clean and organized work environment.
  • Maintain all patient information and records.
  • Maintain all pertinent documentation as required.
  • Participate in departmental quality improvement activities in order to continually improve and evaluate pharmaceutical care as reflected by operational and patient care indicators.
  • Practice appropriate hand hygiene.
  • Provide and received information (via telephone, person-to-person, or written) to clinical personnel, patients and appropriate caregivers. Refer all drug information questions to Staff Pharmacists.
  • Use computer skills to process and maintain all patient information and records.
  • Additional responsibilities within the realm of licensure, as requested by the Medical director or designee.  

Qualifications

  • Completion of high school diploma strongly preferred.


Licensure, Certifications, and Clearances:

  • Act 34 Criminal Clearance
