Description
Community HealthChoices (CHC) is Pennsylvania's managed care long term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid.
Responsibilities:
- Maintain linkages with the mental health community and other relevant health and human services; provide interagency liaison, especially liaison designed to meet the goals of the county Unified Systems Program.
- Meet weekly with case managers in group supervision and provides individual supervision as needed.
- Review and assist consumer/patients with the established appeal procedure in the event that there is disagreement with the service plan.
- Review referrals for Case Management, assigns and coordinates ICM/RC caseloads.
- Attend agency and state trainings as appropriate. Identify training needs and ensures presentation of appropriate training as necessary.
- Develop and implement program policies and procedures.
- System contract for quality control reports.
- Maintain overall responsibility for day-today program operations and development as per the approved county Case Management Plan and the Unified Systems Program.
- All Service Coordinator Supervisors will be scheduled at a minimum for a one week rotation of on call every seven weeks. Supervisors will adhere to the practices as outlined in the Service Coordination On Call guidelines.
- Participate in recruiting, hiring and training of intensive case managers to implement intensive case management service as described in the county plan and Unified Systems Program.
- Review Case Manager's activities as measured against the 55% face-to-face and community-based standards.
- Evaluate all intensive case management staff on a regular basis.
- Provide review of case manager's documentation, including assessments, service plans, progress reports, correspondence and other related monitoring and reporting documentation for cases assigned to the Intensive Case Manager/Recourse Coordination.
- Supervise up to nine (9) Intensive Case Managers.
- Monitor the on-call rotation schedule for ICM's.
- Routinely meet with clinical programs for clinical performance review and progress monitoring of Case Manage interventions.
- Assist Case Manager and the consumer/patient in advocating for desired services both within the clinical treatment services and within the community.
Qualifications
A master's degree in social work, psychology, rehabilitation, activity therapies, counseling or education and 3 years mental health direct care experience; or A bachelor's degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, education and 5 years mental health direct care experience, 2 of which shall include supervisory experience; or A bachelor's degree in nursing and 3 years mental health direct care experienceLicensure, Certifications, and Clearances:
