The BSU Case Manager II will provide blended case management services to the individuals and families who struggle with substance use disorder. Help the client/family gain access to, monitor and coordinate the needed mental health and related services and will assists clients/families to acquire and maintain a quality lifestyle in the community. The elements of practice must A) reflect clinical expertise B) reflect cultural awareness and acknowledge diversity, C) be based upon the principles of CSP & CASSP, D) demonstrate a knowledge of case management skills community and service access and an understanding of the clinical implications of such, E) include an ability to develop collegial relationships with community providers within and external to WPIC, F) demonstrate leadership capabilities in incorporating the mission of the Division into positive growth and services to the population.


The ideal candidate will have strong communication skills as you will be communicating with various providers as well as the community. The shift for this role is first shift.



  • Able to participate in case management intake process in initial assessment of newly referred consumers.
  • Able to comprehensively assess the holistic needs of the consumer and to develop plans and goals needed to facilitate independent functioning and to decrease dependency.
  • 50% of the case manager's time will be engaged in billable, consumer-oriented activity.
  • Assume the formal role of mentor to newly hired or junior staff. Also able to serve as a resource, provided consultation or training to peers, Institute colleagues or other community providers.
  • Maintain regular contact with consumers during inpatient hospitalizations. Offer ongoing support to the consumer and consultation to the treatment team. Seek to be involved in discharge planning within parameters described in the Pennsylvania Bulleting.
  • Provide case management services to a caseload of seriously and persistently mentally ill consumer/patients. Caseload size will be determined by allowable provisions specified in the Pennsylvania Bulleting.
  • Through regular contact with consumers and provider, monitors consumer's participation and satisfaction in all services used by consumers on their caseload, and determine if the individual is progressing and if specific services continue to be needed and are appropriate.
  • Attend and may provide agency and state training as appropriate or as required.
  • Identify basic medications and side effects and to supervise the consumer in correctly filling his/her pill box and perform pill counts. Identify common adverse reactions to medication and report to the physician.
  • Function autonomously and utilize appropriate judgment in the analysis and solving of sophisticated problems.
  • Assist consumers to access and to coordinate appropriate mental health and related services.
  • Provides advocacy for consumers, including participation in 302 and other involuntary commitment proceedings, guardianship and custody proceedings.
  • Document and complete assessments, service plans, progress reports, case correspondence, service logs and other required monitoring and reporting documentation as appropriate and on a timely basis.
  • Plan and goal are to indicate consumer self-determination and to be based on the consumer's strengths, needs, interests and cultural preferences.
  • Assist consumers to obtain and maintain culturally appropriate basic living needs and skills such as: housing, food, medical care, recreation, education and employment, through referral and assertive follow-up, advocacy and problem resolution.
  • Work within the context of the interdisciplinary treatment team and/or as a member of an identified program/module and as such is expected to attend staffing, consultations and case conferences on assigned consumers. In some cases the treatment team may consist of the case manager and the physician, meaning the case manager is required to be able to work autonomously, but in a manner consistent with the consumer's treatment plan.
  • Facilitate or co-lead small groups of consumers/or significant others to increase understanding of the consumers issues or to improve coping skills and performance of activities of daily living.



  • Bachelors Degree in Social Work, Psychology or Counseling required. 
  • Bachelors in other fields of study may be considered with coursework of at least 12 credits in human services and/or relevant mental health experience. 
  • 2 years of mental health direct care experience or a registered nurse with mental health experience.
  • Experience must relate directly to the special population to be served or the unique skill to be performed.
  • May in the course of their duties be in rooms where medications are stored or may transport medications when they transport patients. Transportation of medication is limited from one department to another.




Licensure, Certifications, and Clearances:

  • ACT 31/126 required, but will be categorized dependent on the population the BSU Case Manager II will serve.  To be obtained within 90 days from hire or transfer date.
  •  Applied Suicide Intervention Skills Training - ASIST certification to be obtained within 6 months from hire or transfer date

  • Basic Care Management/Resource Coordination - BCMRC certification to be obtained within 2 weeks from hire or transfer date
  • Children's Case Management/Resource Coordination - CCMRC certification to be obtained within 2 weeks from hire or transfer date

  • Crisis Prevention Institute (Non-Violent Crisis intervention) - CPINV certification to be obtained within 6 months of hire or transfer date

  • Comprehensive Crisis Management (CCMC)  - to be obtained within 2 months of hire or transfer date

  • Cardiopulmonary Resuscitation (CPR) – to be obtained within 30 days of hire or transfer date

  • Automotive Insurance

  • Driver's License

  • Act 33 with renewal

  • Act 34 with renewal

  • Act 73 FBI Clearance with renewal
